Carotid injury during transsphenoidal surgery is a dreaded event for neurosurgeons. Preoperative knowledge of the anatomy, essential intraopertive tools (Doppler) and a methodical, deliberate action plan by the surgical team is necessary to avoid and mitigate this morbid surgical complication.
