My daughter wanted to have lunch with me today. On weekdays, that is almost never possible. But today was the first of January, and I promised her I would come. Sharp at 2.00 pm. At 1.30, just as I was about to leave, a 30 year old man, Mr. Mahesh (name changed), was rushed in with severe chest pain. He was having a massive heart attack. There was no choice. An emergency angioplasty followed. Over the next ninety minutes, he was given a new lease of life. His angio pics are posted below. My daughter missed her lunch with her dad on the first day of the new year. But Anya (name changed), the kindergarten going daughter of Mr. Mahesh, will now have her father with her on her birthdays, school days, and all the important moments of her life. Not all doctors are saints. But the wives and children of doctors who work in emergencies, they are the true angels. Happy new year 🎉🎉🎉

Humble day for a cardiologist… A patient of 44 years came with an axillary chest pain and scapular pain. (I have indicated the pain sites in the accompanying picture). He has had diabetes for the past three years and also has a family history of heart disease. He is not a smoker and doesn’t have any other coronary risk factors. His ECG, ECHO and TMT were normal. Because the pain was atypical and non-exertional and his non-invasive testing was normal, I prescribed medication and sent him home. He came back again with more severe pain at the same site after a month, and again, the ECG and Troponin were normal. I reassured him once more. He came back the next day (last Saturday) with the same complaints, but this time I suggested doing an angiogram to investigate the persistent pain. What I found was a 90% lesion in the PLV artery. I stented the vessel, and he experienced total relief from his chest pain. Physicians and cardiologists should be willing to modify their diagnosis when the initial line of management does not provide clinical benefit to the patient. We have normalised drinking #alcohol so much that 1. We don’t mind our heroes and heroines boozing. 2. We don’t frown at girls falling unconscious on the roads inebriated. 3. We ridicule those who don’t drink in parties. 4. We have so many memes about how much pleasure is in drinking alcohol. 5. We strongly believe that ‘moderate’ alcohol consumption is not bad. 6. We frown at parties with no alcohol. 7. We feel that governments earn legitimately from liquor sales and that it is alright. 8. We boast about the new year alcohol sales from our city/state. Now seventh class students drinking alcohol?? Let’s face it. We all saw this coming. #Shame
