💡 30y old male referred to our centre for severe mitral regurgitation. Classic Barlow valve with mixomatous degeneration as shown by 2D/3D TOE: ✔ Huge annulus dilatation (48 mm - intercommissural) ✔ Main lesion: wide P2 prolapse, small P3 prolapse ✔ Secondary lesion: A1 prolapse ✔ Mitro-aortic angle: 110° ✔ SAM-risk: ++++ Surgical approach in right mini thoracotomy: ✔ P2 resection ✔ A1 neochordae implantation ✔ Complete annuloplasty (38 mm ring) ✅ No residual MR ✅ No SAM ✅ 1 cm coaptation lenght Stunning resolution with Vivid E95 #GEcasesTOE and excellent result! #echofirst
