Why RIP works. And why you should try this at home today! Here are 3 slides by Gregor Leibundgut, and one video from our lab. You don’t need to be a CTO operator to try this. It works bec. once the shaft is disrupted, contrast leaves the balloon slowly maintaining tamponade. After the GEC is loaded, the wire introducer tool becomes the new hub of the 🎈shaft!

Proof Of Concept! 🔥 Yesterday Rich_cathRT(CI)☢️ had an idea, and today we put it to the test! Pt is a 78 year old with prior CABG and a known wide aortic root. The referring MD had difficulty engaging the LM. This is our attempt using the hybrid JL/EBU guide! Worked like a charm.😀

Since we are all sharing, what do you make of these impressive coronaries? And fistulae! The case was referred to a colleague. Our plan was to occlude the RCA → PA connection with plugs in a retrograde manner, but we were unsuccessful. (The fistula exits just above the pulmonic valve, and there is no easy way to intubate it from that side.) The patient is doing better with beta-blockers.

Ex Vivo Grenadoplasty! ( Or, what pressure does it take to rupture an NC balloon?) 👉🏼 We used an OPN indeflator to see how we could go on a 4 mm BSC NC balloon. Note the point of rupture! The NC 🎈remains intact! 🤔 But the hypotube is completely mangled. The video speaks for itself. Best viewed with the volume up!
