77 yo with NSTEMI, severe ICM (EF 20%), severe LFLG AS (AVA 0.8/MG 29), MV CAD (calcified diffusely diseased prox-mid RCA, dLM-mLAD). Turned down surgery. BAV first- improved MG to 20, Impella assisted single access PCI ✅TAVR outpatient next. #cardiotwitter AtlantiCare
