In a recent interview, Sapir, a senior fellow at the Manhattan Institute and co-author of the 2025 HHS review of pediatric gender medicine, detailed a systemic lack of rigor and transparency within the field. According to Sapir, the research landscape is frequently compromised by professional and ideological interests: "The people who do studies in this area are typically gender clinicians... who believe in it for sometimes very ideological reasons, sometimes financial reasons. Sometimes they just believe that they're helping kids.” But the result is studies that are "very short term," have "high dropout rates," and fail to track long-term harms like bone density loss or cognitive impacts. Perhaps most striking is the suppression of unfavorable data. Sapir points to a multi-million dollar NIH-funded study led by Johanna Olson-Kennedy: "[She] had the mental health outcomes from puberty blockers on the kids in her study, and she chose not to publish the results because they were unimpressive.
