On Wednesday, Dame Donna Ockenden will unveil the findings of the largest maternity inquiry in NHS history, examining the deaths and injuries of hundreds of babies and the experiences of more than 2,400 families. The inquiry into stillbirths, neonatal deaths, brain injuries and maternal deaths within Nottingham University Hospitals NHS Trust comes as police carry out a wider criminal investigation into its alleged maternity failings. Filmmakers and friends Pip Bennett-Warner and Pippa Vosper have set to highlight the issue based on harrowing true stories depicting what some Black women have gone through in their new film 22+1. The NHS has launched a rapid investigation into maternity failings which the Secretary of State has pledged to enact. Please be aware this report does deal with the distressing topic of baby loss.

The BBC has launched what it's calling a "fast-track investigation" into why a racial slur - made during its coverage of the BAFTA film awards - wasn't edited out. John Davidson, who has Tourettes, shouted the slur, involuntarily as Delroy Lindo and Michael B Jordan were presenting an award. But the BBC didn't bleep it. Ashna Hurynag reports.

Three people were killed while 8 others were critically injured after a fire broke out in a five-storey residential building in southeast Delhi's Govindpuri area in the early hours today. One thing that stood out to me was that the neighbours made a significant effort to help. People even used hanging sarees to climb down and save their lives. I believe that if neighbours help each other in this manner, many such tragedies can be prevented or their impact reduced. However, whenever such incidents occur, inquiries are announced, but the investigation reports never come out. In Palam, nine people died. In March, it was said that an investigation would be conducted and that it might take a month or a month and a half. Now the fourth month is underway, and no report has been released on what happened in Palam, why it happened, or who was responsible. Then there was the fire in Vivek Vihar in which nine people died. It was said that the investigation report would be available within a week, but that report has also not been released. A week ago, there was a fire in Malviya Nagar in which 23 people died. It was announced before the media that the report would be submitted within three days. More than a week has passed, and that report has not come out either.

It says the 'exceptional decision' was made to clear the site because it poses a 'fire risk', adding that it will only take such action in 'truly exceptional circumstances'. The 150-metre long pile of rubbish provoked outrage and experts say it posed a risk to the environment. One man who was arrested as part of an investigation into the dumping of waste has since been released pending further investigation.
