4 A.M. STEMI call. 45-year-old male with acute anterior STEMI activated through the STEMI pathway. Coronary angiography showed complete thrombotic occlusion of the LAD. Initial ballooning restored flow, and a drug-eluting stent was deployed successfully… or so we thought. Seconds later came one of the true nightmares in interventional cardiology: The stent was physically delivered. Deployment was confirmed. But fluoroscopy showed… no stent in the lesion. The LAD still looked unstented. Immediate full fluoroscopic search of the patient’s body was performed. No lost stent was seen anywhere. The table? Negative. The sheath? Negative. The guiding system inside the body? Negative. Unfortunately, stent boost was unavailable at that moment, and IVUS was not accessible. Multiple vigorous flushes of the guiding catheter failed to reveal the missing stent. At that point, the concern escalated dramatically — embolized stent? Lost in the system? Somewhere unseen? Decision was made to retrieve the entire system and physically inspect everything outside the patient. Still nothing. Finally, as a last attempt, the guiding catheter was carefully cut in small external segments. And there it was. The entire undeployed stent was trapped inside the OUTSIDE portion of the guiding catheter — beyond the fluoroscopy field — firmly stuck despite repeated flushing attempts. The relief in the cath lab at that moment was indescribable. A new stent was then successfully deployed with excellent final angiographic result. Keys to access 🔑 Don’t get frustrated 🔑 Understand the anatomy and how to troubleshoot location of vessel 🔑 Don’t rush! 🔑 Don’t be ashamed to ask for help/give up the needle Equipment needed ⭐️ Use a 21G (micro puncture) needle for any access. ⭐️ 21G needle works with an 0.018 wire. Can consider micro wire, nitrex, or platinum plus among others ⭐️ 6F sheath usually used (sheath size is by inner diameter vs guide is outer) hemostasis As a fellow spend at least the first month not only learning access but also HEMOSTASIS Put the radial hemostasis band on yourself. Understand patent hemostasis! Pull the groin sheath yourself. An educational rota burr entrapment case. When a burr is stuck, we pull on the burr. If that doesn’t work, then we think of complex manoeuvres: cutting the burr, removing Teflon sheath, guide extensions, parallel wiring/ballooning and ping pong guides etc… But we often forget the simple stuff. The 0.014 tip of rotawire is larger than 0.009 shaft that contains the burr. This prevents distal vessel injury but can help retrieve a stuck burr. If wire is intact, bring wire back to burr, but this can only be done with brake defeat on (this is often forgotten!!!). Once the tip is at the burr, just pull! Works in many cases of stuck burr. Here a case of LAD rotablation. Unrecognised calcific distal LMS disease, led to burr advancement and entrapment. Tugging the burr didn’t help. So brake defeat on and then pull the rota wire. Burr came out.
