American Surgeon shows the actual letter from UnitedHealthcare DENYING a patient in emergency condition from receiving care “This is a woman who was in the emergency room with pulmonary embolisms” “I think we all knew this would happen. I had another patient come in and share with me that UnitedHealthcare denied her inpatient's day. So this is a patient who had shortness of breath and some chest pain, and she just knew that something wasn't right in her body. She had a family history of blood clots and she'd had a deep flap surgery a couple of weeks ago. She went to the hospital and they saw her and they found that she had a life threatening condition known as pulmonary embolisms. So she was admitted to the hospital and taken care of really well by the doctors there. And they ordered all the right things. After a couple of days, she was discharged. She got a letter from UnitedHealthcare explaining that they didn't agree with the level of her care and that they would not cover it. So I'm gonna share some of the language of that letter with you, and I want you to know that my patient that we talked about previously who had her surgery denied had almost exactly the same letter shared. So there's some troubling things in this letter. I think this term is really interesting. United is saying they reviewed the request for inpatient admission. So let's all just pause and consider that. This is a woman who was in the emergency room with pulmonary embolisms, and the doctor wasn't really requesting anything. They were saying this patient needs to be in the hospital. But an insurance company sees this as a request, and that's part of this prior auth environment that we're living in. So I think it's important as patients and as physicians to just acknowledge that this is our reality now. Someone can think that there's a good medical decision for you and can write orders and wanna do the right thing for you, but your insurance company is seeing that as a request and deciding whether or not they wanna do it. One of the criteria that this insurance company used to decide whether or not to accept or deny this request was whether it's medically necessary. And it's so interesting that we're letting insurance companies and the doctors who work for insurance companies determine what's medically necessary and not just the doctor in front of the patient in the emergency room. So this is a really bold statement from UnitedHealthcare for my patient. They say you did not have to be admitted as an inpatient to the hospital for this care. I think we all need to just reflect on that. An insurance company is telling a patient and her doctor that they disagree with the plan of care to keep that patient safe. I know that this is boiling down to whether it's an inpatient admission or an observation admission, and that's really about money. But what I wanna point out to you is they're making medical decisions. This insurance company is actually weighing in and disagreeing with a doctor who made a medical decision to admit this patient for her safety. So this specific sentence, when a doctor or facility treats a patient above the recommended level of care, we cannot cover it. What the heck? That's what we do. We go above and beyond as physicians. It's clear that insurance companies don't, and they're actually saying it here.”

They won't do that because they don't want to extend one year of subsidies to make sure that people have affordable health care. So you're going to have people who are deciding, do I spend $500 to go to a doctor, or do I buy groceries? And those same people are the people you guys are saying are living in a safety jacuzzi. And some of these people are your voters. More than half the people in the marketplace live in Republican congressional districts. A part of me feels like you're getting the government you voted for, but the human in me can see the humanity in these people, more so than I've seen other people be able to extend and say they don't deserve to be without health care.

I work at a hospital in an emergency room and part of my job is to deal with insurance stuff and the hospital I work for now has added this paper that I have to give to old people and try to make them sign who are in the emergency room and sometimes people like can go to get physical therapy and stay at a nursing home from the emergency room and we just hold them there until it's time to go based on their insurance. But they want me to give these poor old people this piece of paper that says you may have to pay $3,500 a day for your stay if your insurance doesn't cover it. And it feels so dirty and I feel like a shitty human for having to give these things out and yeah, what do you do? What do you do? I think I know some things about health care. And I'm just going to tell you that the only reason they think it's a pipe dream is because they have been bending backwards to accommodate the big pharmaceutical industry and their money, the big insurance industry and their money, the big hospital industry and their money. Once you decide you're not taking that money and you are not going to be beholden to them, this is the best way forward to provide every single person health care in this country.

The first story says that even though we got rid of a king, there is still a caste system in America, a pecking order of who makes decisions That if somebody doesn't look like or think like you or practice religion the same way you do, they must be a threat to your way of life, and they need to be put in their place. That is how Donald Trump thinks about America. Make America great again by putting the people in charge even if they don't know what the hell they are doing. But here is the thing. That story is not new. It is the oldest story in the book. It is not even uniquely American. For most of human history, that is the way society has worked. For somebody on top and somebody on bottom. There were lords and peasants. For a long time, that story of caste and privilege and concentrated power was the law of the land here in America. If you look like me, you were likely treated as property. If you were a woman, or a white man who did not own property, you could not vote. But from the very start, there was another story, born of this nation's true revolutionary spirit, a story that says, we the people means what it says, that all of us are included, that we are not subjects, but citizens, defined not by race or religion or gender or sexual orientation, but by our commitment to a common creed and a willingness to accept not just privileges, but responsibilities that come with that citizenship.

It is about making sure that we can have the likes of the people that have been elevated into Trump's administration, who we know are not qualified and skipping over qualified black men that yes, I may disagree with but we can all say that Byron Donalds as well as Tim Scott are more qualified than every single person that has been put into this cabinet. That is the type of life that they want us to have in this country is one in which even if you are mediocre, even if you are unqualified but you are white, that will be enough.

So most people get a REAL ID attached or integrated into their driver's license, correct?" Brase: "Yeah. Although I'd like to say it sort of the other way around. What they have is a real ID, and the federal government is allowing the state to use it for driving. Driving purposes. We will allow you to use it as a driver's license, but we require it for getting on a plane, we require it for federal buildings, we require it for nuclear facilities. And according to the law, we can require it for anything that we want that the Secretary determines. "And so... It's a federal ID, and it could be a federal ID that you— that would be required for you to open a bank account, to access medical care, to go to college— or rent a hotel room or rent a car or get married or anything, because that's what the law says. The law says that its official purposes include any purpose that the Secretary of Homeland Security shall determine. So any purpose. And right now, the American people don't realize the system that's being set up here. "I think the other thing to understand about REAL ID is that the federal government is creating a facial recognition system for the country through the REAL ID, because the real ID requires a biometric, starting with the face. And so, you know, the idea that the government is going to— has claimed a right to collect and capture the coordinates of your body for their control should be anathema to every American. "But Americans don't realize this. They don't realize that the picture that's being taken is a digital picture for the creation of a facial recognition system. A government facial recognition system where all of people's faces are being put into a database. And I'm going to add here because a lot of people, they go to the DMV and they don't understand the picture that's being taken. They just think it's a picture, right? They don't, they don't understand. But what they do understand is that when they go to fly, they're being asked to stand in front of a camera there. So what I'd like to say about that is, so this whole facial recognition system is being created through the REAL ID and about 56% of the American public have their faces in the government database already."

So most people get a REAL ID attached or integrated into their driver's license, correct?" Brase: "Yeah. Although I'd like to say it sort of the other way around. What they have is a real ID, and the federal government is allowing the state to use it for driving. Driving purposes. And so, one of the things I just like to say to get people to understand how this is working is that when you have a REAL ID, the REAL ID is a federal ID masquerading as a state driver's license. People see it looks just like their driver's license before they got a REAL ID, but they do notice that there's a star on it. So it's a— It's a whole masquerading process here which is keeping the American people from understanding what's happening. "And essentially the, the federal government has hijacked the state DMVs by virtue of the fact that the legislators around the country, have decided to bow to the federal government to give up the constitutional rights and the 10th Amendment rights of the people and the states and hand it over to the federal government. "So the federal government has nationalized identification and driving privileges. Or you could say that they have nationalized identification through the REAL ID and then told the states, yeah, you can use it as a, as a driver's license. We will allow you to use it as a driver's license, but we require it for getting on a plane, we require it for federal buildings, we require it for nuclear facilities. And according to the law, we can require it for anything that we want that the Secretary determines. "And so... It's a federal ID, and it could be a federal ID that you— that would be required for you to open a bank account, to access medical care, to go to college— or rent a hotel room or rent a car or get married or anything, because that's what the law says. The law says that its official purposes include any purpose that the Secretary of Homeland Security shall determine. So any purpose. And right now, the American people don't realize the system that's being set up here. "I think the other thing to understand about REAL ID is that the federal government is creating a facial recognition system for the country through the REAL ID, because the real ID requires a biometric, starting with the face. And so, you know, the idea that the government is going to— has claimed a right to collect and capture the coordinates of your body for their control should be anathema to every American. "But Americans don't realize this. They don't realize that the picture that's being taken is a digital picture for the creation of a facial recognition system. A government facial recognition system where all of people's faces are being put into a database. And I'm going to add here because a lot of people, they go to the DMV and they don't understand the picture that's being taken. They just think it's a picture, right? They don't, they don't understand. But what they do understand is that when they go to fly, they're being asked to stand in front of a camera there. So what I'd like to say about that is, so this whole facial recognition system is being created through the REAL ID and about 56% of the American public have their faces in the government database already."

Yes, indeed, this is lawlessness by any standard. Even by banana republic standards, this is still lawlessness. Your country has a constitution, it has a government, it has a police service, and it has a ruling party. I am sure you can see that some of the people there are actually wearing ruling party T-shirts. It is lawless regardless of whoever does it. It is an embarrassment to South Africa as a country, what you are doing and what you are encouraging people to do. Your country has an immigration service. If people are in your country illegally, they should be arrested and deported through lawful processes. You do not go around destroying property, tearing down markets, and attacking people. It is illegal regardless of whoever does it. It is not illegal because I have said so. It is illegal because the laws of your country make it so. This is vigilantism, pure and simple, and it is tainting the reputation of South Africa, not only across Africa but across the world. If you have got satellite television in your home, you can see that these actions are being reported everywhere. It is not good for your country. This kind of barbarism undermines the rule of law, fuels division, and damages South Africa’s standing as a constitutional democracy. It is the actions of a few that are tainting the reputation of many. The average South African is not mindless like this. They respect the law, and they respect the fact that among them, in their communities, there are people from other countries. If those people are in the country illegally, you report them and the law takes its course through proper processes of arrest and deportation. You're making that sound as if it's reasonable and it's not okay. The president making a unilateral determination in his own mind that somebody has broken the law without providing, by the way, any proof of that. And then directing an investigation into that person is. Calling for that is that is not that is not appropriate. Okay.
