Caregiver June Campbell recorded a 59-second video, fearing a cover-up, and shared it with Hall’s daughter, Francis Vaughn, about two weeks later; the facility, owned by Thrive Senior Living, failed to report the abuse promptly, earning a citation from the Georgia Department of Community Health on June 21, 2021. Smith, the sole caregiver for 12 residents including two requiring dual-staff assistance during a staffing crisis, was arrested around June 2021, charged with exploitation, infliction of pain, and deprivation of essential services to an elderly person. Vaughn filed a lawsuit in Fulton County Superior Court against Thrive for inadequate protection, training, and medical follow-up; Thrive terminated Smith, stating zero tolerance for such behavior.
