Lack of impulse control results in black Americans making poor decisions. These poor decisions have other races of people looking at black Americans with a side eye. These poor decisions also in many cases result in black Americans losings, jobs, relationships, or their freedoms. And then many black Americans complain about the system without ever seriously looking at any of this. Gambling companies are finding loopholes to entice vulnerable people back onto their platform - even when people have decided they want to stop gambling and self excluded on BetStop. This is what happens when you leave gaps that can be exploited by what is known to be a predatory industry. And it shows what will happen if the Government opts for a partial ban on gambling ads, rather than a full ban. The late Labor MP Peta Murphy and her committee report gave us the blueprint for change, it's time for the government to implement the Murphy Report recommendations in full. If you want to read more, this is from Peta Murphy's Foreword to the committee report: "Online gambling companies advertise so much in Australia because it works. Online gambling has been deliberately and strategically marketed alongside sport, which has normalised it as a fun, harmless, and sociable activity that is part of a favourite pastime. Gambling advertising is grooming children and young people to gamble and encourages riskier behaviour. The torrent of advertising is inescapable. It is manipulating an impressionable and vulnerable audience to gamble online. Australia’s largest professional sporting codes and broadcast media were largely in lockstep with their advertising and sponsorship partners in the gambling industry during this inquiry to oppose further restrictions. The Committee shares the concerns of many witnesses and the community that the status quo is leading to a generation of young Australians who view gambling and sport as inextricably linked. This is concerning, not only because of the potential increase in Australians experiencing gambling harm, but also about changes to the culture of sport. Australia would be diminished if sport was to be so captured by gambling revenue that providing an opportunity for betting came to be seen as its primary purpose. ‼️Dont ignore these early signs of Ovarian Cancer. It is possible to detect #ovariancancer if we are picking up the symptoms early by listening to women and investigating with a transvaginal USS and CA125 blood test **It’s important to know a CA125 and a ultrasound scan can come back normal. So if the tests are normal - STILL continue to track the symptoms and depending on the symptoms, risk factors etc a referral to gynaecologist might be needed for a further investigations such as CT, MRI or a Ovarian biopsy (a procedure where a small sample of tissue from the ovary is removed to check for abnormalities, most commonly to diagnose or stage ovarian cancer)** … thankfully my patient’s Ovarian Cancer was picked up early and now is doing well - post treatment Hope this helps

Check out the unusual gait pattern worsened by levodopa in this case of a PARKIN gene related Parkinson's. This is why we call it Parkinson's diseases plural as there are so many different manifestations, with or without known gene mutations. Nice case by Mehta and colleagues in Movement Disorders Clinical Practice. Key Points: - 26 year old Parkin gene-related early-onset Parkinson's. - Paradoxical response to levodopa, dopamine worsened dystonia. - Amantadine seemed to help. - Complaint was tightness of lower limbs when walking. - Had heterozygous PRKN mutations, w/ a 'missense pathogenic mutation in exon 7 (c.850G > C (p. Gly284Arg) and a deletion mutation in exon 3 (241 bp),' - 'Forward walking was characterized by bilateral hip flexion and abduction, flexed knees, ankle inversion and plantar flexion rebound during the stance phase on the left foot and reduced arm swing bilaterally.' My take: Some of these PARKIN and genetic PD cases may respond to anticholinergics or dopamine agonists, however in this case this was not helpful. The improvement w/ amantadine was humbling and unexpected. The authors suggest to us to try amantadine, if dopamine is worsening dystonia or gait in the setting of PARKIN. 'Parkinsons diseases, every day we learn something new.' A close look at the video suggests the amantadine helped however did not resolve the challenge. DBS may be a reasonable next step to consider. #Parkinsons

The review cites over 300 sources, most of which are peer-reviewed scientific studies, and digs into evidence from older and better-designed studies that have not yet been brought into the conversation about puberty suppression. To our knowledge, this is the most thorough review of GnRH agonists, the drugs now marketed as “puberty blockers,” to date. Three Takeaways Yes, there IS enough research. No more claiming that the research on puberty suppression just isn’t there. No more calling for more clinical trials or urging caution until more studies are conducted on vulnerable children. A massive body of research on GnRH agonists already exists—you’ve just been using the wrong search terms. The research shows that “puberty blockers” are really, really dangerous. Particularly urgent concerns for adolescents include loss of bone mineral density and increased risk of osteoporosis; potential for decreased IQ and other cognitive deficits; increased risk of depression and suicidal thoughts; and stunted sexual and reproductive development. GnRH agonists can also affect the heart, thyroid, digestive system, urinary tract, muscles, eyes, and immune system, and cause chronic pain. A lot of the side effects are irreversible. Osteoporosis, dementia, and diabetes are irreversible. Suicide is irreversible. Chronic pain can last for years, or forever. And major health events like heart attack, stroke, and pituitary tumors can cause problems for the rest of a person’s life. Not to mention that treating the other side effects of GnRH agonists may be extremely difficult, painful, and costly, and require other medications with other side effects.

There’s no doubt these drugs are incredibly effective for weight loss. But the high doses currently being used might be too much of a good thing. For example, high-dose GLP-1 receptor agonist use has been linked with a near doubling in the risk of suicidal behavior and a tripling in the risk of depression, as well as a two-fold increase in the risk of age-related macular degeneration. However, these drugs may still have utility when used strategically. According to Dr. Ben Bikman (Benjamin Bikman), micro-dosing GLP-1 receptor agonists intermittently may be a beneficial strategy to manage hunger, reset eating behaviors, and encourage healthier dietary patterns. Specifically, Bikman proposes that using low doses for about 90 days could fundamentally shift one's relationship with food. Do you remember the case of the 'frozen addict' from recreational drug use (MPTP). Is there a 'new frozen addict syndrome' caused by methcathione (ephedrone). YES. This psychostimulant leads to release of brain catecholamines. Check out this case and video by Thayler and Gurevitch which responded to amantadine and check out the link to manganese. Key points: - Methcathione (ephedrone) can be used as a recreational drug and use has been documented in Eastern Europe and other world locations. - This chemical is synthesized by oxidation of ephedrine or pseudoephedrine w/potassium permanganate. - It is usually given IV and the 'parkinsonism' is thought to be caused by manganese. - It is unlike the MPTP frozen addict as in this case levodopa does not work to treat. - The authors point out that the syndrome tends to be more symmetrical than in idiopathic PD; the clinician should look for bradykinesia, dystonia, and gait impairment. - Check out this 41 year old case w/ a normal flurodopa PET and an abnormal MRI. - He was treated with a 5-day course of IV amantadine followed by oral amantadine. My take: It is humbling to think about the history of a recreational drug which provided one of our best overall animal models of PD (the MPTP model). Now we are observing another form of recreational drug induced parkinsonism, and this one seems to be related to manganese. It is interesting in this case to observe the response to IV and oral amantadine; which as a therapy has multiple mechanisms of action including on dopamine, acetlycholine and glutamate pathways. In the USA, we have not used IV amantadine so this will be a new trick for us. One important aspect of this case was that the clinicians resisted the urge to 'dismiss the person' as an addict or as functional case. Similar to Bill Langston's experience with the MPTP frozen addict, these authors persisted, made the diagnosis and designed and implemented a treatment. BRAVO. American Surgeon shows the actual letter from UnitedHealthcare DENYING a patient in emergency condition from receiving care “This is a woman who was in the emergency room with pulmonary embolisms” “I think we all knew this would happen. I had another patient come in and share with me that UnitedHealthcare denied her inpatient's day. So this is a patient who had shortness of breath and some chest pain, and she just knew that something wasn't right in her body. She had a family history of blood clots and she'd had a deep flap surgery a couple of weeks ago. She went to the hospital and they saw her and they found that she had a life threatening condition known as pulmonary embolisms. So she was admitted to the hospital and taken care of really well by the doctors there. And they ordered all the right things. After a couple of days, she was discharged. She got a letter from UnitedHealthcare explaining that they didn't agree with the level of her care and that they would not cover it. So I'm gonna share some of the language of that letter with you, and I want you to know that my patient that we talked about previously who had her surgery denied had almost exactly the same letter shared. So there's some troubling things in this letter. I think this term is really interesting. United is saying they reviewed the request for inpatient admission. So let's all just pause and consider that. This is a woman who was in the emergency room with pulmonary embolisms, and the doctor wasn't really requesting anything. They were saying this patient needs to be in the hospital. But an insurance company sees this as a request, and that's part of this prior auth environment that we're living in. So I think it's important as patients and as physicians to just acknowledge that this is our reality now. Someone can think that there's a good medical decision for you and can write orders and wanna do the right thing for you, but your insurance company is seeing that as a request and deciding whether or not they wanna do it. One of the criteria that this insurance company used to decide whether or not to accept or deny this request was whether it's medically necessary. And it's so interesting that we're letting insurance companies and the doctors who work for insurance companies determine what's medically necessary and not just the doctor in front of the patient in the emergency room. So this is a really bold statement from UnitedHealthcare for my patient. They say you did not have to be admitted as an inpatient to the hospital for this care. I think we all need to just reflect on that. An insurance company is telling a patient and her doctor that they disagree with the plan of care to keep that patient safe. I know that this is boiling down to whether it's an inpatient admission or an observation admission, and that's really about money. But what I wanna point out to you is they're making medical decisions. This insurance company is actually weighing in and disagreeing with a doctor who made a medical decision to admit this patient for her safety. So this specific sentence, when a doctor or facility treats a patient above the recommended level of care, we cannot cover it. What the heck? That's what we do. We go above and beyond as physicians. It's clear that insurance companies don't, and they're actually saying it here.”
