In furtherance of His Excellency President Bola Ahmed Tinubu, GCFR, Bola Ahmed Tinubu, Renewed Hope Agenda, and in line with our national commitment to achieving Universal Health Coverage (#UHC), I am pleased to provide an update on Nigeriaโ€™s progress in expanding access to quality and affordable health insurance. Out-of-pocket payments remain the dominant source of health care expenditure for most of our people, translating into higher costs and lower standards of accountability for providers. When the Federal Ministry of Health and Social Welfare Federal Ministry of Health, NIGERIA signed the Renewed Hope Ministerial Performance Monitoring Bond with its detailed key performance indicators, we set ambitious 2030 targets that are now within reach, one of which is national health insurance coverage. Under the leadership of Mr President, all macroeconomic reform and cross-sectoral policy direction are driven by one objective: ensuring that every Nigerian can actualize their aspirations while accessing affordable, quality care. This people-centred approach defines the comprehensive reforms now reshaping our health sector. As we modernize infrastructure across primary, secondary, and tertiary facilities in all local government areas, our record-breaking progress in national health insurance enrollment is aligning demand with Nigeriaโ€™s rapidly expanding supply of human resources and infrastructure. Nearly 120,000 health workers have been trained since 2023, and 2,500 doctors, nurses, midwives, and community health extension workers have been recruited to strengthen frontline services. Over the past 16 months, 4,000 health personnel have also been added in Federal Tertiary Hospitals to close workforce gaps. In 2024, Nigeria broke its enrollment record with more than 2.4m new people insured, bringing total coverage to about 20m Nigerians. With new policy tools in motion, we are on course to reach 44m by 2030. This is the surest path to reducing out-of-pocket expenditure, which still represents about 70% of total health spending. Social health insurance schemes now account for 90% of all enrollments nationwide. A major constraint in care quality has been the low capitation fee for enrollees. For years, the capitation stood at โ‚ฆ750 per person. We have doubled it to โ‚ฆ1,450 to ensure providers are properly equipped to deliver consistent, high-quality care. Fee-for-service rates have also been increased by 380%, based on actuarial evidence aligning cost-reflective rates with quality requirements. The Federal Government Government of Nigeria has also introduced the One Hour Referral Authorization Code, issued by the insuring entity to the provider to confirm referral approval. By shortening this process to 1 hour, we are eliminating unnecessary delays and ensuring that patients move quickly from primary to specialist care. To strengthen accountability, I have directed the National Health Insurance Authority NHIA NIGERIA to commence covert mystery shopping to monitor providers and ensure that enrollees are not denied treatment. Undercover monitors are already observing processes in facilities nationwide to ensure that insured Nigerians receive timely and dignified care. The evidence is clear. From fewer than 10m hospital visits in all of 2023, more than 46m visits were recorded by the second quarter of 2025. Reforms in the Basic Health Care Provision Fund Basic Health Care Provision Fund and the NHIA are delivering measurable gains in patient confidence and service utilization. As enrollment expands, benefits multiply. Wider adoption of health insurance enhances quality, strengthens accountability, and protects households from financial hardship. These achievements reflect President Tinubuโ€™s vision of a health system that serves all Nigerians, regardless of income or status. We will continue to modernize infrastructure, strengthen the workforce, and sustain reforms until every Nigerian is covered and cared for. Public Health Agency of Canada releases first report on psychological manipulation of citizens๐Ÿฉบ๐Ÿ‡จ๐Ÿ‡ฆ Under Chief Medical Officer Theresa Tam, the Public Health Agency of Canada has launched a behavioural science office and released a report on persuasion tactics for compliance, raising ethical concerns about transparency in how taxpayer money is used to influence public behaviour. Canadaโ€™s public health agency (Health Canada and PHAC) has established a behavioural science office (BeSciO) and has recently released its first report detailing its persuasion of public compliance under the guidance of Chief Medical Officer of Canada, Theresa Tam (Canada's CPHO). Tam, who also serves on various World Health Organization committees, has been heavily scrutinized for this conflict of interest in recent years. The report starts with a land acknowledgment and outlines the Public Health Agency of Canadaโ€™s progress in establishing BeSciO from April 2021 to March 2023. Launched in Spring 2021, BeSciO emphasizes the importance of human behaviour in public health and supported the government's pandemic response with critical advice and evidence. The report defines behavioural science (BeSci) as a multidisciplinary field that studies human behaviour and decision-making by integrating psychology, neuroscience, and social sciences. It emphasizes the importance of understanding psychological, sociocultural, and systemic factors that influence decision-making, enabling practitioners to create effective solutions that impact behaviour-based outcomes. Itโ€™s noted in the report that BeSciO has โ€œjoined forces with the Office of the Chief Public Health Officer, the World Health Organization, the Organisation for Economic Co-operation and Development and the Behavioural Insights Team, to name only a few.โ€ As such, itโ€™s important for the public to be prepared for the โ€œcrazy ideasโ€ coming down the pike, as stated by WHO director-general Tedros Ghebreyesus (Tedros Adhanom Ghebreyesus) during a strategic roundtable on behavioural science for better health in 2022. Tam confirms collaboration with the World Health Organization (WHO), utilizing their behavioural insights tool to monitor compliance with public health measures. The new unit says it is โ€œindebted to the incredible staff of BeSciO, as well as PHACโ€™s Data, Surveillance and Foresight Branch (formerly Corporate Data and Surveillance Branch).โ€ The report is signed by Heather Devine (Heather Devine ๐Ÿ‡จ๐Ÿ‡ฆ) and Dhurata Ikonomi (Dhurata Ikonomi ๐Ÿ‡จ๐Ÿ‡ฆ), but the rest of the behavioural science team remains unclear. Notably, while partner Impact Canada lists its behavioural insight team, neither Devine nor Ikonomi are included, suggesting the government employs behavioural scientists in various, less transparent roles beyond public health. BeSciO was formed after a โ€œneeds assessmentโ€ was conducted in April 2021, and then Minister of Health Patty Hajdu, who is said to have โ€œrecognized the critical role of human behaviour in public health.โ€ โ€œCreating BeSciO introduced an innovation and experimentation function within the Branch to support PHAC's revitalization as a world-leading, data-driven public health organization,โ€ it says. It would appear that one of their first tasks was the May 2021 memo titled โ€œTesting Behaviourally-Informed Messaging In Response To Severe Adverse Events Following Immunizationโ€ where messaging and communication around COVID-19 vaccine injuries needed to be framed in a way that did not compromise the government's safe and effective marketing slogan. Faced with mounting distrust in public health after its grotesque failures in pandemic handling and response, in its first year, BeSciO aimed to address โ€œpriority areas such as vaccine confidence, public health measures, combatting mis/disinformation, borders measures, and public health communications to inform key pandemic response activities. In its second year, BeSciO expanded its focus to new areas that emerged as critical to PHACโ€™s mandate, including climate change and infectious disease, mental health, and antimicrobial resistance.โ€ The new office highlights the growing global use of behavioural science to improve health outcomes by addressing human decision-making tendencies. It aims to predict responses to policies, identify barriers to health-promoting behaviours, design aligned programs, and evaluate interventions for better outcomes. In its first two years, it completed 11 research projects, responded to over 50 advice requests, recruited 10 fellows, conducted 10 training sessions, expanded its community of practice by 308 members, and published 63 newsletters on topics including COVID-19, public trust, mental health, and climate change. It further outlines how it came after the WHO developed the behavioural insights tool โ€“ the organization that says โ€œOne of the most critical elements of reducing virus transmission is public behaviour.โ€ BeSciO plans to use its research to gauge Canadians' intentions regarding public health measures, studying misinformation and public trust in PHAC. Future project areas include antimicrobial stewardship, childhood immunizations, mask-wearing, mental health, and climate change. They will apply the IMPActS model to shape perceptions and behaviours, which involves identifying policy challenges, mapping targeted changes, prototyping solutions, testing effectiveness, and scaling successful strategies. Notably, their efforts include supporting routine childhood immunization programs during the pandemic through collaboration with the Vaccine Behaviour and Confidence Team of PHAC. The use of behavioural science to nudge compliance often amounts to coercion, occurring subliminally without the public's awareness, undermining personal autonomy and trust. It raises ethical concerns about experimenting on individuals without consent and emphasizes the need for transparency from the agency about these techniques and their impact on public behaviour, particularly given the millions in taxpayer funding involved. First Lady Bibi Asifa Bhutto Zardari Briefed on Proposal to Establish JPMC Comprehensive Stroke Center First Lady Bibi Asifa Bhutto Zardari, MNA, was today briefed on the vision and implementation roadmap for the proposed Jinnah Postgraduate Medical Centre (JPMC) Comprehensive Stroke Center by Dr. Syed Fazal Zaidi, Professor of Neurology at the University of Toledo, internationally renowned vascular and interventional neurologist and Founding Director of the JPMC Comprehensive Stroke Program, along with Mr. Mushtaq Chapra, Chairman of the Patient Aid Foundation. During the briefing, Dr. Zaidi highlighted the growing burden of stroke in Sindh, noting that the province, with a population of over 55 million, records an estimated 500 stroke cases every day, including around 50 patients with severe strokes requiring highly specialised care. He emphasised the urgent need to strengthen the public healthcare system by establishing a Comprehensive Stroke Center capable of providing timely, life-saving treatment. The First Lady was informed that while NICVD and SICVD provide valuable care for patients with ischemic stroke, Sindh currently lacks a Comprehensive Stroke Center capable of managing the full spectrum of stroke care, including both ischemic and hemorrhagic strokes, complex neurovascular emergencies, advanced neuro-interventional procedures, neurocritical care and specialised rehabilitation. Dr. Zaidi said the initiative is being led through the Pakistan Stroke Initiative, a US-based non-profit organisation established to advance stroke care in Pakistan, in partnership with the Patient Aid Foundation, JPMC and the Government of Sindh. The partnership aims to establish Sindhโ€™s first public-sector Comprehensive Stroke Center, which will provide advanced stroke care, specialist training and research while serving as the hub of a province-wide tele-stroke network. As the country bearing the worldโ€™s highest malaria burden, accounting for approx. 39.3 percent of malaria related deaths among children under five, deployment of the R21 Matrix M vaccine marks a major public health milestone.
