Tall patient, 6’ 8”, prox radial stick, 100 cm catheters wont reach. I have 125 cm JL/JR, but RCA was ant-sup takeoff and needs AL. So you wont find all the catheters in the lab with 125 cm. Yesterday, I did the first BASILICA in my new place after starting an electrosurgery program over the past few months. A few things I learned so far: • Didn’t have an Astato 300 cm, only 180 cm, workable, but takes a few more steps. • Confianza Pro 12 wasn’t as effective as Astato 20 for traversal. I’d use the short Astato to cross, then cross the leaflet with the microcatheter and exchange for a 300 cm Confianza for the flying V • Corsair Pro can still get the job done without piggyback, a 0.035 Torquer is needed • Smaller gooseneck snares can work if you don’t have the ideal size • Biplane fluoroscopy definitely helps, but not having it isn’t a dealbreaker, just makes things a bit more challenging in extreme angles. Hydraulic Exchange Technique: super useful in complex PCI/CTO work. Quick way to get a microcatheter off a short wire without a 300cm wire, trapper, or trapping balloon. Connect an indeflator to the microcatheter, pressurize to 20 atm (it drops constantly, so keep bumping it back up), and just open the Touhy and microcatheter will come out right away. The hydraulic force pushes the catheter back on its own while keeping the wire in position. I don’t do it over a hydrophilic wire as it can jump forward and cause a tip perforation. Coronary fat embolization for late coronary perforation. I did a routine DK-crush bifurcation PCI. 6 hours later, pt is hypotensive with a large effusion. Relook angio showed diagonal tip perforation that wasnt there at the end of PCI. Prolonged balloon tamponade failed. Coils unavailable. Treated with autologous fat embolization, an effective bailout technique. • Small 0.5 cm groin incision • Subcutaneous fat harvested using forceps into small bowl • Fat mixed with saline • Loaded into a 2 cc syringe • Using a microcatheter (Turnpike/Finecross/Export, etc.), to inject fat into the perf site. • Final angiography confirmed complete seal. Learn from my pitfalls: Severe iliofemoral tortuosity during a MitraClip case. Right iliac: the guide wouldn’t cross over Bayliss wire then Amplatz Super Stiff. Tried the left side instead, very tortuous too. 24F DrySeal went through fine, but the guide still wouldn’t follow. Went back to the right, swapped for a Lunderquist wire. Left the DrySeal sitting in the left iliac/IVC to help straighten things out. Right before pushing the guide through, pulled the DrySeal back to the left iliac and it finally advanced. Lesson: in tortuous vessels before you think about switching to the other side, don’t stop at one stiff wire. Escalate: Bayliss, Amplatz Super Stiff, Safari, Lunderquist, Meier. The theory of a contralateral sheath straightening the IVC might be helpful too. Another idea is to put a 28 F dry-seal then advance the mitraclip through it? Curious to hear how others have handled similar tortuosity? "It's about honesty, the basis is to be honest, even if its not nice for patients....We will cope with this" Eva Schumacher-Wulf Dear Friends, It is with a heavy heart that we learned this weekend that one of our longtime faculty members and dear friends has passed away. Eva Schumacher-Wulf was an exceptional patient advocate, a patient scientist who always contributed to the field, never afraid to challenge the status quo, and a fierce fighter for what was right.I (Gil) first met Eva in San Antonio in 2021, I believe, and that was where I truly saw the impact she was making within the patient advocate community in Europe. She was someone who believed we all deserved to be part of the same Conversations. Fast forward three years, Eva became one of our faculty members for our first in-studio Breast Cancer discussion, produced together with Mirrors of Medicine and she flew to Belgium just to come and record. Alongside our other faculty, she thoughtfully and wisely represented the patient perspective, never making the conversation about herself or what she was personally going through. She believed in the science and in doing what was right for all patients.Eva was always happy to lend her voice to ensure the patient perspective was heard at oncology congresses. She reported live on behalf of OncoAlert at ESMO in 2024, continuing her unwavering commitment to the community.Above all, Eva was a wonderful person—selfless, kind, and deeply principled.I saw Eva one last time at ASCO, where we spoke briefly. As always, I left our conversation saying, "See you at ESMO, and don't forget—we still have to get together in San Antonio." In 2023 Kemi Badenoch hosted an Iftar (the evening meal during the month of Ramadan) at Lancaster House After she delivered a keynote speech to Muslims and non-Muslims launching a group to try and make Britain the global Center of Islamic Finance There was also a call to prayer which includes the phrase, "I testify that there is no god but God" and the where the video cuts off is the phase, "I testify that Muhammad is a messenger from God" -Both of these phrases have been criticised, even though it's the same call to prayer that is sounded the world over and hasn't changed for 1,404 years- Immediately after the call to prayer, prayers would have been held, which includes men and women praying separately, segregated, though in the same space For both this Lancaster House event and the Trafalgar square event, both publicly owned spaces, people are invited to attend. And content from both is shared online, making both events even more public and visible The only significant difference in Kemi Badenoch’s stance between the Lancaster House Iftar in 2023 and the Trafalgar Square Ramadan event in 2026 is her change in attitude She previously accepted public Muslim religious events as normal, but later framed them as incompatible with British cultural norms The shift reflects a deliberate change in her political stance, revealing how thin her principles are and how willing she is to pander to racists and bigots in a declining Conservative landscape just to cling to relevance and build a base of support As a former Conservative voter its tragic to see the depths to which the Conservative party will go

There have been rumors going around that I'm not a doctor, a fake MD, faking my credentials, haven't worked in neurosurgery, etc. This was started by "Dr Idz" Dr Idz and has been pushed around by the likes of Dr Alo, Layne Norton, PhD and Stan Efferding among others. These rumors are categorically false and defamatory, and my lawyers have sent letters to this effect to many of the above named individuals. So let's just set the record straight. I am a medical doctor with a post graduate medical degree from the Royal College of Surgeons, class of 2013. This is an internationally recognized and accredited medical school whose degrees are equivalent to the MD degree granted in America, Canada, Australia, and many other countries. I completed my sub-internships at Duke University Medical Center in America in plastics, reconstructive, and maxillofacial surgery, as well as at Beaumont Hospital in Dublin in neurosurgery and orthopedic surgery. I was an intern at Cork University Hospital, and at St Vincent's Hospital in Dublin for my second year. After this I took some time off for family reasons as well as to do humanitarian work in the refugee camps in Bangladesh helping the Rohingya refugees who escaped a genocide in Burma in 2017. For nearly 6 years now I have been a practicing physician in Australia, predominantly as a neurosurgical registrar at Sir Charles Gairdner Hospital in Perth, and more recently in my own private metabolic health practice. I have an active medical license and I'm registered with the Australian licensing board, AHPRA, and this can be found easily on their website by searching for for my name on their registry. So for those who have been spreading these lies and false rumors, now you know the truth, and if you continue to spread these lies you will be committing libel and defamation and may get a letter from my lawyers as well if you're not careful. In August of last year, I heard Secretary Kennedy speak about Good Energy during a press conference. I ordered the book, dove in head first, and after 7 days, my daughter needed to come off her afternoon medication she was taking in attempt to manage bipolar disorder. This was the first improvement in 6 years! Then, one year ago today, we were watching the Nutrition Roundtable to see what more we could learn from Casey Means, MD on our family’s metabolic health journey. Little did we know, our lives were about to change forever. I think our whole world might have stopped for a minute when Chris Palmer, MD spoke. We changed her diet to a ketogenic diet, and within days, she was an entirely new person in all of the best ways. This has been the best year of her life, and it’s been the best year for our family, too. She thanks me almost every single day, but today - it’s Dr. Palmer day in our house and she wanted to thank all of you herself! ❤️ So - we all thank you, from the bottom of our hearts.

Conquered NYC... Once Again, Islamic Supremacists Seize Control of Times Square for Ramadan In a show of power and control, Islamic supremacists took to the streets of New York City streets to show the world they had conquered another Western city. On March 10th, thousands of Muslims descended on Times Square to "celebrate" the month-long Islamic holiday of Ramadan. (Not one media outlet is questioning WHY they are not celebrating in their mosques) The sharia-adherent event, held on the 3rd night of Ramadan, was called “A Historical Dawah Event, Taraweeh in Times Square.” First comes Dawah, then comes Jihad Dawah is the Islamic concept of inviting individuals to embrace Islam. Muslims set- up Islamic information tables at the event and “invited” non-Muslims to embrace Islam. While many might see this “invitation” as harmless, the “call” or “invitation” to Islam is a legal requirement before Muslims wage jihad against non-Muslims. The Koran speaks of Dawah – the Call to Islam. “We do not punish until We send a messenger.” (Koran 17:15). Tafsir Ibn Kathir, which legally defines every verse in the Koran, states this verse means Muslims cannot punish non-Muslims until they “give a warning” to them by inviting them to Islam. This is Dawah Islamic preachers also performed the controversial adhan, which declares the supremacy of Allah of Islam and sends the strongest message to Christians that Allah is superior to the Trinity and Jesus. Islamic supremacists use psychological warfare on those they aim to conquer. For example, they know it’s important to make people in the West accustomed to hearing the call to prayer or celebrating Islamic holidays. If heard or recognized often enough, it no longer seems out of place and instead becomes as normal as hearing church bells ringing or seeing Christmas decorations. Meanwhile, in some Islamic lands, it is illegal for Churches to ring bells, as per Islamic law (Sharia). The same concept applies to seeing women walking the streets in burkas or watching as the halal sections of grocery stores slowly occupy more space. *** Parodius Demo Update *** After seeing my good friend Pyron amazing Parodius artwork , I could not let that go to waste. Its a crying shame Konami's shooters did not make it to MD at the time , time to rectify that (in time). The video shows some of the progress so far. Stage 1 scrolling using the streaming scroller from lufthoheit to automatically manage the tile management for the scrolling rather than using different sets etc. Its great when I can grab bits of code and apply them to new projects, it speeds things up !! Lufthoheits new stage is coming along well also and has its first enemies in , hopefully show something of that soon. The VRAM view is to the right , we will have plenty of vram left for objects without resorting to much manual management it seems. ~ 1/2 vram left. It looks like the biggest challenge for this level is the Cat Ship mid boss - the end boss seems easier in comparison for a few reasons. I've analysed the way it was done on PCE and SNES and will come up with the best option for MD. More to come on that soon. At present we will target mainly assembly , so performance won't be an issue . I'm not sure we need a sprite multiplexor for > 80 sprites , but we will use one anyway just to not have regrets/changes later on and for the Hint color effects the multiplexor can do also. We have an A level musician volunteering also for those concerned about the audio side of things - it wont dissapoint I'm sure. As this is now my 3rd / 4th project it will be done as time permits so no gaurantees on timelines sorry - we are all hoping to do well by the community however. #SGDK #Konami #Parodius #MegaDrive #SegaGenesis
