Aspiration thrombectomy in STEMI with a heavy thrombus burden, maybe it’s time to take another look? Had a 37F with a huge RCA thrombus. Tall patient, 6’ 8”, prox radial stick, 100 cm catheters wont reach. I have 125 cm JL/JR, but RCA was ant-sup takeoff and needs AL. So you wont find all the catheters in the lab with 125 cm. If you dont have long catheters, this is an easy way to make your catheters longer. Works well for both guides and diagnostics! Watch for your fingers! Yesterday, I did the first BASILICA in my new place after starting an electrosurgery program over the past few months. A few things I learned so far: • Didn’t have an Astato 300 cm, only 180 cm, workable, but takes a few more steps. • Confianza Pro 12 wasn’t as effective as Astato 20 for traversal. I’d use the short Astato to cross, then cross the leaflet with the microcatheter and exchange for a 300 cm Confianza for the flying V • Corsair Pro can still get the job done without piggyback, a 0.035 Torquer is needed • Smaller gooseneck snares can work if you don’t have the ideal size • Biplane fluoroscopy definitely helps, but not having it isn’t a dealbreaker, just makes things a bit more challenging in extreme angles. Hydraulic Exchange Technique: super useful in complex PCI/CTO work. Quick way to get a microcatheter off a short wire without a 300cm wire, trapper, or trapping balloon. Connect an indeflator to the microcatheter, pressurize to 20 atm (it drops constantly, so keep bumping it back up), and just open the Touhy and microcatheter will come out right away. The hydraulic force pushes the catheter back on its own while keeping the wire in position. I don’t do it over a hydrophilic wire as it can jump forward and cause a tip perforation. I use a workhorse wire. Video below Credit to J Aaron Grantham JAG for teaching me such valuable skills! Coronary fat embolization for late coronary perforation. I did a routine DK-crush bifurcation PCI. 6 hours later, pt is hypotensive with a large effusion. Relook angio showed diagonal tip perforation that wasnt there at the end of PCI. Prolonged balloon tamponade failed. Coils unavailable. Treated with autologous fat embolization, an effective bailout technique. • Small 0.5 cm groin incision • Subcutaneous fat harvested using forceps into small bowl • Fat mixed with saline • Loaded into a 2 cc syringe • Using a microcatheter (Turnpike/Finecross/Export, etc.), to inject fat into the perf site. • Final angiography confirmed complete seal. Join us at TCTME+ in Sep 2026. Will have a lot of great #CHIP and complications discussions. Learn from my pitfalls: Severe iliofemoral tortuosity during a MitraClip case. Right iliac: the guide wouldn’t cross over Bayliss wire then Amplatz Super Stiff. Tried the left side instead, very tortuous too. 24F DrySeal went through fine, but the guide still wouldn’t follow. Went back to the right, swapped for a Lunderquist wire. Left the DrySeal sitting in the left iliac/IVC to help straighten things out. Right before pushing the guide through, pulled the DrySeal back to the left iliac and it finally advanced. Lesson: in tortuous vessels before you think about switching to the other side, don’t stop at one stiff wire. Escalate: Bayliss, Amplatz Super Stiff, Safari, Lunderquist, Meier. The theory of a contralateral sheath straightening the IVC might be helpful too. Another idea is to put a 28 F dry-seal then advance the mitraclip through it? Curious to hear how others have handled similar tortuosity? "It's about honesty, the basis is to be honest, even if its not nice for patients....We will cope with this" Eva Schumacher-Wulf Dear Friends, It is with a heavy heart that we learned this weekend that one of our longtime faculty members and dear friends has passed away. Eva Schumacher-Wulf was an exceptional patient advocate, a patient scientist who always contributed to the field, never afraid to challenge the status quo, and a fierce fighter for what was right.I (Gil) first met Eva in San Antonio in 2021, I believe, and that was where I truly saw the impact she was making within the patient advocate community in Europe. She was someone who believed we all deserved to be part of the same Conversations. Fast forward three years, Eva became one of our faculty members for our first in-studio Breast Cancer discussion, produced together with Mirrors of Medicine and she flew to Belgium just to come and record. Alongside our other faculty, she thoughtfully and wisely represented the patient perspective, never making the conversation about herself or what she was personally going through. She believed in the science and in doing what was right for all patients.Eva was always happy to lend her voice to ensure the patient perspective was heard at oncology congresses. She reported live on behalf of OncoAlert at ESMO in 2024, continuing her unwavering commitment to the community.Above all, Eva was a wonderful person—selfless, kind, and deeply principled.I saw Eva one last time at ASCO, where we spoke briefly. As always, I left our conversation saying, "See you at ESMO, and don't forget—we still have to get together in San Antonio." Had I known that would be the last time we would speak, our conversation would have been very different. We will always miss Eva, and our hearts go out to her family, friends, and everyone whose lives she touched. In 2023 Kemi Badenoch hosted an Iftar (the evening meal during the month of Ramadan) at Lancaster House After she delivered a keynote speech to Muslims and non-Muslims launching a group to try and make Britain the global Center of Islamic Finance There was also a call to prayer which includes the phrase, "I testify that there is no god but God" and the where the video cuts off is the phase, "I testify that Muhammad is a messenger from God" -Both of these phrases have been criticised, even though it's the same call to prayer that is sounded the world over and hasn't changed for 1,404 years- Immediately after the call to prayer, prayers would have been held, which includes men and women praying separately, segregated, though in the same space For both this Lancaster House event and the Trafalgar square event, both publicly owned spaces, people are invited to attend. And content from both is shared online, making both events even more public and visible The only significant difference in Kemi Badenoch’s stance between the Lancaster House Iftar in 2023 and the Trafalgar Square Ramadan event in 2026 is her change in attitude She previously accepted public Muslim religious events as normal, but later framed them as incompatible with British cultural norms The shift reflects a deliberate change in her political stance, revealing how thin her principles are and how willing she is to pander to racists and bigots in a declining Conservative landscape just to cling to relevance and build a base of support As a former Conservative voter its tragic to see the depths to which the Conservative party will go

There have been rumors going around that I'm not a doctor, a fake MD, faking my credentials, haven't worked in neurosurgery, etc. This was started by "Dr Idz" Dr Idz and has been pushed around by the likes of Dr Alo, Layne Norton, PhD and Stan Efferding among others. These rumors are categorically false and defamatory, and my lawyers have sent letters to this effect to many of the above named individuals. So let's just set the record straight. I am a medical doctor with a post graduate medical degree from the Royal College of Surgeons, class of 2013. This is an internationally recognized and accredited medical school whose degrees are equivalent to the MD degree granted in America, Canada, Australia, and many other countries. I completed my sub-internships at Duke University Medical Center in America in plastics, reconstructive, and maxillofacial surgery, as well as at Beaumont Hospital in Dublin in neurosurgery and orthopedic surgery. I was an intern at Cork University Hospital, and at St Vincent's Hospital in Dublin for my second year. After this I took some time off for family reasons as well as to do humanitarian work in the refugee camps in Bangladesh helping the Rohingya refugees who escaped a genocide in Burma in 2017. For nearly 6 years now I have been a practicing physician in Australia, predominantly as a neurosurgical registrar at Sir Charles Gairdner Hospital in Perth, and more recently in my own private metabolic health practice. I have an active medical license and I'm registered with the Australian licensing board, AHPRA, and this can be found easily on their website by searching for for my name on their registry. So for those who have been spreading these lies and false rumors, now you know the truth, and if you continue to spread these lies you will be committing libel and defamation and may get a letter from my lawyers as well if you're not careful. Thank you! #doctor #weightloss #diet #nutrition #weightlossjourney

In August of last year, I heard Secretary Kennedy speak about Good Energy during a press conference. I ordered the book, dove in head first, and after 7 days, my daughter needed to come off her afternoon medication she was taking in attempt to manage bipolar disorder. This was the first improvement in 6 years! Then, one year ago today, we were watching the Nutrition Roundtable to see what more we could learn from Casey Means, MD on our family’s metabolic health journey. Little did we know, our lives were about to change forever. I think our whole world might have stopped for a minute when Chris Palmer, MD spoke. We changed her diet to a ketogenic diet, and within days, she was an entirely new person in all of the best ways. This has been the best year of her life, and it’s been the best year for our family, too. She thanks me almost every single day, but today - it’s Dr. Palmer day in our house and she wanted to thank all of you herself! ❤️ So - we all thank you, from the bottom of our hearts.

Mayor of London Sadiq Khan at the ramadan Tent Projects Iftar in Trafalgar square on March 16th 2026, "One of the reasons why we are the greatest city in teh world" "I want to say to my non-Muslim friends who are here tonight" "To my Jewish friends, to my Christian friends, to my Hindu friends, to my Sikh friends, thank you" "This is what allyship looks like" Your reminder that the Ramadan Tent project have held iftars - the evening meal during the month of ramadan for many years, including under four Conservative Prime Ministers with absolutely no issue whatsoever: ➡️ 2015 under David Cameron ➡️ 2019 under Theresa May ➡️ 2022 under Boris Johnson ➡️ 2024 under Rishi Sunak Trafalgar square is a hub of universal events. For example on April 3rd, The Passion of Jesus, a free annual open-air play, will take place between 12:00 PM and 3:15 PM There are 50+ events in Trafalgar square every year, the main religious cultural ones include: ➡️ Diwali on the Square (Hindu, Sikh, Jain) ➡️ Eid in the Square (Islamic celebration) ➡️ Jewish Heritage Month and Hanukkah events ➡️ Chinese New Year (East Asian cultural festival) ➡️ St Patrick’s Day Festival (Irish culture) ➡️ Black on the Square (Black culture & heritage) ➡️ Pride in London (LGBTQ+ culture, often ending there) In addition to which there are multiple cultural showcases too Everyone is entitled to disagree with whatever they wish from other people's chosen way of life, but Kemi Badenoch's suggestion that one faith should be singled out and not allowed to hold an event at Trafalgar square is sad

Conquered NYC... Once Again, Islamic Supremacists Seize Control of Times Square for Ramadan In a show of power and control, Islamic supremacists took to the streets of New York City streets to show the world they had conquered another Western city. On March 10th, thousands of Muslims descended on Times Square to "celebrate" the month-long Islamic holiday of Ramadan. (Not one media outlet is questioning WHY they are not celebrating in their mosques) The sharia-adherent event, held on the 3rd night of Ramadan, was called “A Historical Dawah Event, Taraweeh in Times Square.” First comes Dawah, then comes Jihad Dawah is the Islamic concept of inviting individuals to embrace Islam. Muslims set- up Islamic information tables at the event and “invited” non-Muslims to embrace Islam. While many might see this “invitation” as harmless, the “call” or “invitation” to Islam is a legal requirement before Muslims wage jihad against non-Muslims. The Koran speaks of Dawah – the Call to Islam. “We do not punish until We send a messenger.” (Koran 17:15). Tafsir Ibn Kathir, which legally defines every verse in the Koran, states this verse means Muslims cannot punish non-Muslims until they “give a warning” to them by inviting them to Islam. This is Dawah Islamic preachers also performed the controversial adhan, which declares the supremacy of Allah of Islam and sends the strongest message to Christians that Allah is superior to the Trinity and Jesus. Islamic supremacists use psychological warfare on those they aim to conquer. For example, they know it’s important to make people in the West accustomed to hearing the call to prayer or celebrating Islamic holidays. If heard or recognized often enough, it no longer seems out of place and instead becomes as normal as hearing church bells ringing or seeing Christmas decorations. Meanwhile, in some Islamic lands, it is illegal for Churches to ring bells, as per Islamic law (Sharia). The same concept applies to seeing women walking the streets in burkas or watching as the halal sections of grocery stores slowly occupy more space. These seemingly small concessions are strategic wins in Islamic supremacists’ larger battle to conquer. Check out my coverage of Ramadan in NYC from 2023:

$( #WednesdayMotivation #Wednesdayvibe Hump Day Garnacho #GetBetrayed #NHS75 Horizon ) Statement No. 02 The Story of the death of King Salman Al Saud and the clinical death of Mohamed Ben Salman owing to coronavirus. Warning to Prince Mohammed Bin Salman.. Imam Mahdi Nasser Mohammed Al-Yamani 23-Jumada II-1443AH 26-January-2022AD 07:07 AM (According to the official time of Mecca Mother of Towns) __________ Warning to Prince Mohammed Bin Salman In the name of Allaah, the Most Gracious the Most Merciful. O His Royal Highness Prince Mohammed Bin Salman Bin Abdul Aziz, I bear witness to Allaah and all mankind and jinns that I command you to set all my Saudi and Non-Saudi supporters from the prisons of KSA, all at once without any delay in implementing the order Perhaps Mohammed Bin Salman would like to say: "And who are you, O Nasser Mohammed Al-Yamani, to issue an order to His Royal Highness Mohammed Bin Salman to give orders to release your Saudi and Non-Saudi supporters from the prisons of Saudi Arabia?" Then, Nasser Mohammed Al-Yamani replies to you and says: " I am indeed Imam Mahdi, Allaah's caliph on the whole world. I swear by Allaah the Great who will achieve His purpose, if you did not obey the command of Allaah's caliph Al-Mahdi Nasser Mohammed Al-Yamani, Allaah would do to you what He has done to your Father; thus He Sends against you His firm plan, COVID, so it strikes you down into a clinical death as it did to your father; leading to your own death! So your highness and superiority in Earth would not benefit you and you will never find besides Allaah a helper or protector, then He makes you an example for whoever disobeys the order to obey His caliph on the whole world Perhaps Mohammed Bin Salman would like to say: "Do you believe the rumors about the death of my father? Nay, they are just conjectures. O Nasser Mohammed Al-Yamani, did not you claim that you do not say about Allah conjectures that do not avail anything against the truth?" Then, Allaah's caliph Al-Mahdi Nasser Mohammed Al-Yamani replies to you and says: "O Mohammed Bin Salman, Allaah's caliph on the worlds is not of those who speak using expectations, predictions, or guesses and far is that from being the truth! I swear by Lord of the earth and heavens that the news from Allaah Lord of the worlds confirms the fact that Allaah has stricken down King Salman Bin Abdul Aziz with the so-called Coronavirus. Or do you think that your father was stricken down with corona due to his age and the weakness of his immune system, and you see that you are young and Allaah's firm plan cannot take you down into a clinical death until Allaah pleases then you die? Surely, there is no escape from COVID; Allaah's firm plan, so beware of the planning of Allaah the One the Overpowering! Do not you know that whoever opposes Allaah's caliph Al-Mahdi Nasser Mohammed Al-Yamani has opposed Allaah? And whoever opposes Allaah and His caliph indeed Allaah is severe in Punishment. So who would save you from the severe torment of Allaah if Nasser Mohammed Al-Yamani was truly Imam Mahdi; Allaah's caliph on the worlds?! O man, I do not say I am Allaah's caliph on Yemen only but Allaah's caliph on the whole world. Would not Allaah achieve His purpose if Nasser Mohammed Al-Yamani was truly the Caliph of Allaah who is chosen by Allaah Lord of the worlds? And if I were a liar it would be to my own loss and no harm shall befall you. Also, if I were truthful, Allaah would inevitably hit you with what His caliph Al-Mahdi Nasser Mohammed Al-Yamani,promised you O man, fear Allaah and submit to Allaah's caliph Al-Mahdi Nasser Mohammed Al-Yamani, thus Allaah would increase you in power and honor as I will not break Allaah's promise, in the clear verses of His Book the Great Quran, to whoever fears Allaah the One the Overpowering from among decision-makers, so he sought Allaah's forgiveness and worshipped Him alone no partners Has He. Allaah's has indeed ordered me to increase him in power and honor in line with the promise of Allaah in the clear verses of His Book the Great Quran in the Word of Allaah the Exalted: {وَإِذْ تَأَذَّنَ رَبُّكُمْ لَئِن شَكَرْتُمْ لَأَزِيدَنَّكُمْ ۖ وَلَئِن كَفَرْتُمْ إِنَّ عَذَابِي لَشَدِيدٌ ﴿٧﴾} {And (remember) when your Lord proclaimed: "If you give thanks (by accepting Faith and worshipping none but Allah), I will give you more (of My Blessings), but if you are thankless (i.e. disbelievers), verily! My Punishment is indeed severe."} 14:7 So think and consider and get the plotting of the three out of your head! O Mohammed Bin Salman, What is your thought about Whom Ar-Rahman (Allaah) is with Him?! I swear by Allaah that mankind and jinns will not have power over him, and whoever denied this has tasted a punishment. And we will wait and see who is swifter in planning, the arrogant ones or Allaah? And this challenge is not out of my own accord but it is from Allaah who Knows best what is within yourselves O Mohammed Bin Salman and his soldiers. That is why Allaah has ordered me to say to Mohammed Bin Salman and his soldiers the following: (What is your thought about Whom Allaah is with Him) So it is better for you to obey the command and to turn away from plotting and if you did not I challenge you and all those who want to plot against the Caliph of Allaah the One the Overpowering, and we shall wait and see who is swifter in planning and He has power over everything, the best protector and the best helper. All matters return to Allaah, He knows the treachery of the eyes and whatever is concealed by hearts And I remind and say: Think and estimate and decide and choose what is good for yourself and ask forgiveness for your sin and be one of the thankful and do not be one of the disbelievers, I am for you among the advisors and for all the decision-makers in the peoples of mankind, and I repeat and state that I do not sing to you with poetry or exaggerate without the right in prose, but rather the Caliph of Allaah, the One, the Overpowering, Won't Allaah accomplish his purpose -Glorified and highly Exalted is He above what they say? Whoever worships Allaah alone no partners with Him, Allaah will give him glory to his glory, and whoever disbelieves, Allaah will strip him of his possessions, and he will lose this world and the hereafter. That is the clear loss And Peace be on the messengers and Praise be to Allaah.. Allaah's Caliph and servant Imam Mahdi; Nasser Mohammed Al Yamani Translated by: Deena ADEL ,Bahy Mansour$

*** Parodius Demo Update *** After seeing my good friend Pyron amazing Parodius artwork , I could not let that go to waste. Its a crying shame Konami's shooters did not make it to MD at the time , time to rectify that (in time). The video shows some of the progress so far. Stage 1 scrolling using the streaming scroller from lufthoheit to automatically manage the tile management for the scrolling rather than using different sets etc. Its great when I can grab bits of code and apply them to new projects, it speeds things up !! Lufthoheits new stage is coming along well also and has its first enemies in , hopefully show something of that soon. The VRAM view is to the right , we will have plenty of vram left for objects without resorting to much manual management it seems. ~ 1/2 vram left. It looks like the biggest challenge for this level is the Cat Ship mid boss - the end boss seems easier in comparison for a few reasons. I've analysed the way it was done on PCE and SNES and will come up with the best option for MD. More to come on that soon. At present we will target mainly assembly , so performance won't be an issue . I'm not sure we need a sprite multiplexor for > 80 sprites , but we will use one anyway just to not have regrets/changes later on and for the Hint color effects the multiplexor can do also. We have an A level musician volunteering also for those concerned about the audio side of things - it wont dissapoint I'm sure. As this is now my 3rd / 4th project it will be done as time permits so no gaurantees on timelines sorry - we are all hoping to do well by the community however. #SGDK #Konami #Parodius #MegaDrive #SegaGenesis

🚨 WARNING TEXAS - MUSLIM BROTHERHOOD ALERT! GOVERNOR ABBOTT: Richardson's Muslim Mayor is Literally Defying Your November 2025 Muslim Brotherhood Terrorist Designation! Mayor Amir Omar Just Handed A Ramadan Proclamation to a Notorious Muslim Brotherhood-Tied Organization- While ICE Deported Their Hamas-Linked Leader The city's first Muslim mayor, elected in May 2025, personally presented an official proclamation recognizing Ramadan 2026 as a "blessed month." The proclamation, shared proudly on the Muslim American Society (MAS) Dallas, stated: "Thank you to Mayor Amir Omar and the City of Richardson for recognizing the significance of Ramadan and celebrating the diversity that makes our city stronger." Mayor Omar also proclaimed "Ramadan Mubarak" at events like an interfaith Iftar hosted by the Islamic Association of North Texas (IANT) in Richardson, with videos circulating on social media. MAS is not just any community group- it's a U.S. organization repeatedly documented as a front for the Muslim Brotherhood: 🔺 Founded in 1992 by Muslim Brotherhood (Ikhwan) members; former MAS Secretary General Shaker Elsayed admitted "Ikhwan members founded MAS" and estimated nearly half its active members were Brotherhood-affiliated. DID YOU HEAR THAT? "IT WAS FOUNDED AS THE UNITED STATES CHAPTER OF THE MUSLIM BROTHERHOOD!" 🔺 FBI evidence from the 2007–2008 Holy Land Foundation terror-financing trial linked MAS founders to Hamas-support networks in the U.S. 🔺 In 2014, the United Arab Emirates designated MAS a terrorist organization, grouping it with the Muslim Brotherhood, al-Qaeda, and ISIS. 🔺 MAS literature and past chapter content have included rhetoric blaming U.S. foreign policy for 9/11, antisemitic references (e.g., hadiths calling for violence against Jews), and praise for jihad from former leaders like Esam Omeish. 🔺 Ties extend to convicted figures: former MAS communications director Randall Royer was arrested in 2003 for conspiring with Lashkar-e-Taiba. Just six months ago, in September 2025, ICE detained Marwan Marouf, a top leader at MAS Dallas, due to longstanding federal scrutiny over his family ties (brother-in-law to Bayan Elashi) to the Holy Land Foundation, the largest terrorism-financing conviction in U.S. history, where leaders were found guilty of funneling $12.4 million to Hamas. Despite this recent federal action targeting the heart of MAS Dallas's leadership and its documented Brotherhood/Hamas-linked history, Muslim Mayor Omar chose to partner with and spotlight the very SAME organization by issuing the proclamation through channels they control and celebrate. Just four months after Governor Abbott designated the Muslim Brotherhood (and its affiliates) as foreign terrorist and transnational criminal organizations, banning land purchases, authorizing heightened enforcement, and accusing them of seeking to impose Sharia and "Islam’s mastership of the world." Meanwhile, Mayor Omar decides to PARTNER with MAS Dallas, a repeatedly documented Brotherhood front, by issuing an official city Ramadan proclamation and letting them celebrate it. This is textbook civilization jihad: a Muslim Brotherhood front gets official city endorsement from Richardson’s Muslim mayor, handing them a Ramadan proclamation on their own platforms, while ICE just deported their top leader for ties to Hamas’s largest U.S. terror-financing scheme. It’s not “diversity.” It’s the quiet, incremental normalization of Islamic supremacism under the mask of charity, community, and religious celebration, smack in the middle of North Texas, the very ground zero where federal prosecutors once crushed the Holy Land Foundation network. TEXAS, WHAT IS HAPPENING TO YOUR STATE!? WE ARE ALLOWING RED STATES TO FALL
