Back in the early 80’s there was no managed care, no referrals or pre authorization.. also some insurance companies were non profits back then

I’m all for this - but if insurance plans have to pay out on everything sent to them by doctors without some type of review of medical necessity - wouldn’t insurance premiums go up higher and faster if they are paying higher claims ?

I understand the delays of pre-auth, but if you don't have pre-auth doesn't that mean you have a procedure performed and the insurance can tell you after the fact that it's not authorized? Removing pre-auth doesn't make every procedure authorized by insurance.
