The pattern appears to be a fibrotic NSIP. The question was likely what side of her body does she usually sleep on. My guess is right based on the significantly worse disease on right. She likely has aspiration/reflux (silent or otherwise) overnight.

NSIP pattern..

Fibrotic NSIP. I would ask if she suffers any joint pain for CTD

NSIP. Look for skin changes for CTDs. Ask for history suggestive of Raynaud’s phenomenon.

NSIP pattern. Given the sex and age, I would order work up for CTD.

In such cases, it is recommended to discuss the characteristics with clinicians to develop a better management plan for the patient.

The hrct pattern is consistent with usual interstitial pneumonia
