I am humbled to share a significant milestone in pancreatic surgery: I recently performed the first-ever da Vinci single-port robotic distal pancreatectomy and splenectomy without an additional incision for pancreatic cancer in the United States, approved by both the Institutional Review Board and IDE from the FDA through a 2cm incision inside the belly button leaving no additional scar. This pioneering procedure not only achieved negative margins but also allowed for the thorough harvesting of all local lymphatics, a critical factor in ensuring comprehensive cancer treatment. I am grateful to report that the patient is doing exceptionally well, and it was a privilege to see him in the clinic several days post-operation. The images included illustrate the operating room moments and a snapshot of our follow-up clinic visit with my remarkable ARNP Dana Manzi and myself. This journey underscores the importance of innovation in surgical techniques and the profound impact they can have on patient outcomes. In this operative video, Dr. Cohen demonstrates the telovelar approach for resection of a large hypervascular vermian hemangioblastoma. The interface between the hemangioblastoma and surrounding tissue is developed with coagulation and sharp dissection. As the dissection advances deeper, bleeding risk increases, so any capsular entry is controlled with further coagulation. The tumor is operated on from deep to superficial to protect the floor of the fourth ventricle while identifying larger anterior vessels. Only vessels clearly entering the tumor are coagulated, preserving cerebellar vessels and maintaining hemostasis near the fourth ventricular floor. I’m pleased to report that the first ever surgeon led IDE pilot study of the DaVinci Single Port Robot for Foregut and HPB operations has successfully started at AdventHealth Tampa. Our first patient underwent Cholecystectomy through a 12mm incision hidden in the belly button. The operation went very smoothly without any intra-operative complications. This is an exciting time to be able to offer operations we used to do through a single incision laparoscopically now through a fully robotic approach with incredible precision and visualization. Bare imitation learning without robotic pre-training is an unfairly overlooked area. (it doesn't even have a name) When done right, it has the advantages of programming. It’s reliable, predictable, easy to modify and debug. The only difference is that the source of truth is a small dataset instead of code. And it is glued together by a vision encoder and a transformer model instead of being compiled. It's hard to imagine how Claude Code could appear without a ton of production-grade source code hardened over years of real use. Same for robotic models. Here is the robustness of such a model. It was trained on orange zip ties with no lighting changes.
