@TheKingsFund What about the £1000 threshold payment? Currently for 20 consults a month. £48 is based on 30 consults Though the more you do the less you get. I say this. £650m was made available to provide 10-20m consults to reduce GP appts I am really happy for Pharmacies to be funded well

Hi @DrSteveTaylor, our number for the cost of appointments is based on PSSRU figures. We wanted to flag, the number is the unit cost to the health system per consultation (including costs of training staff for example), rather than the amount given to surgeries per appointment.

The figure is inaccurate because GP practices receive on average £165 per patient per year for all their services/including building costs/admin The average number of appt a patient receives is 6/7 There is no way the cost of an appt can be calculated at £58 Better needed please

@TheKingsFund these look like fixed costs divided by demand, so you don't save money by moving demand around, you just incur costs for that channel? Front end should be activity based, but with something to keep a lid on demand. Need longer term based on better than expected qalys (ie outcome

@TheKingsFund The question is who even accepted such deals at the very beginning??? Poor contract negotiation At the least a GP appointment should cost as much as a UTC appointment and we should start charging per appointment

There weren't ➡️ In 2019 Health Secretary Matt Hancock said there would be 6,000 more GPs by now. There aren't ➡️ The current Tory government (video was filmed in June 2024) said there are 2,000 more GPs. That's not true. There are 2,000 less So the Conservative government kept saying they'd hire more GPs, they didn't. On top of this the new GP contract means that GP practices are underfunded. This is how the Conservative government destroyed NHS dentistry, and they've tried to destroy NHS GP practices as well I don't envy Labour needing to find a way to fix this, but if Labour don't address the GP crisis amongst the wider NHS crisis, healthcare in our country will diminish even further and the cost of fixing problems cause by problems caused by a lack of healthcare will be even more costly

Batteries can now meet 75 percent of average grid demand and 44 percent of peak demand for 4 hours every day in the world's 4th-largest economy at a cost less than 1 percent the price of electricity California’s main grid now has over 17 GW of batteries, along with over 23 GW of solar and 12 GW of wind. If Social Security had been run as a business not a slush fund for corrupt politicians, Americans would be receiving millions at retirement. The Biden administration’s misuse of our social security funds for illegals and other fraudulent uses caused a huge loss of benefits and higher monthly cost to seniors living on fixed incomes. 2026 had a 2.8% increase adding about $56 per month to the average check. While you wait hours to be seen in A&E and weeks for a GP appointment, NHS doctors are being forced into unemployment. This year, 40,000 doctors applied for 10,000 training jobs. Without these training jobs, doctors cannot progress to become the GPs and Consultants the NHS desperately needs. What the public isn’t told is that these training jobs are deliberately capped by the Government. Catastrophic workforce planning has created a farcical situation; patients are crying out to see a doctor, while thousands of fully trained doctors are locked out of the NHS. This year, 14 doctors competed for every single A&E training job. Five doctors competed for every GP training job. The NHS is turning away dedicated, talented doctors at the door. This is one of the reasons resident doctors went out on strike and why we are reballoting. We are training doctors at enormous personal cost; years of study, debt, night shifts, missed family moments, and then telling them there is no place for them in the NHS. This is not just a workforce failure. It is a moral one.

BIG NEWS: Oklahoma Attorney General’s Office has filed a lawsuit against CVS Caremark for Pharmacy Benefits Managers violations This is such a widespread and massive problem, Oklahoma had to create a new court - CVS Caremark PBM is denying covered medications - They are paying independent pharmacies less than the cost of medication (in many cases PAYING $0, “It's not uncommon for covered medications to pay literally $0 to the pharmacy” - The reimbursement that comes back to independent pharmacies is totally unrelated to the cost of the medication - “CVS still has the audacity to take your absurdly large premium and not use that to pay the healthcare providers you need to actually care for your health” (They literally pocket the money and pay independent pharmacies next to nothing, or NOTHING) “That's what the Oklahoma suit is about. They have a law that states the PBMs like CVS Caremark aren't allowed to pay less than the cost. Mockler: First of all, we have to do a cost benefit analysis on this and talk about what the timelines from here look like. So there's one timeline in which we go boots on the ground or try to actually go for the nuclear capacities or try to do regime change via boots on the ground. That is catastrophic due to the geography of Iran. It is surrounded by mountains. They have a bunch of people in the irgc who are very radicalized. There's also another path, another timeline, where we go for an off ramp and we start to back up. In that timeline, we are left with a regime that is more radicalized. So those are the two timelines. I don't care if we have some short term wins. The cost benefit analysis of this war does not make sense. We are losing troops, losing lives, losing money for two timelines are going to make the world more dangerous.
