How can a 99% accurate medical test give you a 9% chance of having the disease if it comes back positive? 🤔 If you are in medicine this is the SINGLE most important diagnostic testing concept to know. Welcome to the difference between specificity and positive predictive value. Sensitivity & specificity are fixed test properties. These do not factor how common a disease is (prevalence) Positive Predictive Value (probability a positive test reflects having a disease) factors in prevalence and is actually more important to clinicians than sens/spec. It is harder to figure out though because we need to have a gestault for how prevalent a disease is for the EXACT patient we are seeing. If you have very low prevalence, even with a great test, most positives are false positives. This is why screening low-risk patients can result in many false positives and harm To master this, just play with the calculator yourself and you will see!!!!👇

💜 One Last Update! 💜 Last week when we were in Minnesota for Clark’s 6-month follow-up for his clinical trial, of all the tests they ran, the one we were both most anxious and most eager to see results from was his brain MRI. For children with untreated Sanfilippo Type A, it’s typical to see 2–5% loss of brain volume each year, along with enlargement of the ventricles (the fluid-filled spaces in the brain). This is one of the most heartbreaking parts of the disease — watching the brain slowly lose tissue and function over time. We were so nervous that Clark’s MRI might show some loss compared to his baseline scan 6 months ago. But the results came back and… ✨ ZERO change. ✨ His brain volume and ventricle size were completely stable — no loss at all. This is such an incredible sign that the treatment may be doing exactly what it’s meant to: clearing the toxic waste from his brain cells and protecting his brain from further damage. If feel that it is SO important for us to share these results as they come because people need to be FULLY aware that these treatments that are out there WORK. These children with Sanfilippo do not need to regress and they DEFINITELY do not have to have their lives cut short. We just need the FDA to say yes. We are so close to a reality where Sanfilippo syndrome is as manageable as a disease like diabetes. P.S. Enjoy this video of our little sunshine having the best time singing!

It’s really rare for me to get sent a video of the outside of the aircraft when I’m zipping along at 40,000ft. This is me yesterday with Stephen White zipping past an old friend in his A320. We were never fully satisfied with the animated components available outside. Some looked good but were hard to customize. Some were easy to use but felt too basic. So 2 months ago, we started building our own. It started as a Framer component library. Then we expanded it to Next.js and MCP, so people can use it with code and AI builders too. Now we’re finally launching the beta on July 2, 11 AM IST. Only for the first 500 users. If you’re building animated websites, this will save you a lot of time. And I think you’re going to love this.
