The outer layer of the hoof is made up of Keratin ( same material that makes up human hair and nails ) so it’s like a hair cut / nail clipping for them:) They can feel it but shouldn’t be painful.

Horses can feel the vibrations made by the hammer as it drives nails into insensitive parts of the hoof, but they should never feel any pain When performing correctly, the process of shoeing causes no pain to the animal.
