The scariest part is not AI making mistakes but humans failing to catch them before they become real world decisions.

That not how the plot of War Games went. The US computer erroneously thought the Soviet Union were launching missiles. The computer was asked to game plan a game of tic tac toe. Only then did it conclude no one could win.

The real danger is not that AI will hallucinate, but that the military has sent the report generated by AI as official information, and someone will look back to find the source after the plane takes off. What is exposed is that the process has collapsed, not just the model.
