Ok... How to mitigate this for dummies. Anytime the slide is locked to the rear, check what's going on in the inside. Round in the chamber? Round in the magazine well (people do forget there is a magazine)? If there is no rounds present, it's safe to let the slide forward

PROVE - POINT in a safe direction - thankfully he wasn’t pointing at his head, REMOVE ammo - literally the second step, OBSERVE Chamber - never done, VERIFY Feeding path - who are we kidding here, EXAMINE bore - thank god he didn’t.
