How did we get here? For crying out loud. 😭!!! Y’all were calling me all sorts of names, saying I was a weirdo. Some of you even said, “They’re just kids.” I was reluctant to post what you’re about to see, but I believe we need to start creating awareness about the extreme things some of these kids are doing and have an honest conversation about how we can tackle the issue. I’ll try to translate some of the Yoruba challenge questions from this video for people who don’t understand Yoruba. Note: The challenge was supposedly meant for grown adults, yet it’s these underage girls in school uniforms participating. so this truth & recon / under cover of night trigger seems to have picked up strange discourse for behaviour i thought was generally agreed as consistent, and the new Halo release has directly changed the music cue, which I can show directly by viewing the original mission data: I checked through to confirm, because Ren here is correct, and it's in line with what I always thought, the game awaits you to either walk into a trigger box or kill any of the designated enemies near the gun turret, either of these will trigger the music cue. Here's a run down of the specific script, the squad AI placements in question, the trigger volume, and a gameplay example. In a busy OPD, chorea is usually not missed because it is rare. It is missed because it looks like restlessness. The patient is sitting in front of you. A finger moves. Then the wrist. Then the shoulder. Then there is a small facial movement. Nothing seems dramatic. But watch carefully. The important thing is this: The next movement is not like the previous one. It changes in timing. It changes in direction. It changes in distribution. There is no fixed rhythm. There is no same movement repeating again and again. That is when I start thinking of chorea. I remember it this way: Tremor repeats. Chorea keeps changing. Now don't rush into a detailed examination. In OPD, five simple things are enough to recognise the phenomenology. 1. Just watch Keep talking to the patient. Watch the fingers, face, shoulders and feet. Mild chorea may simply look like fidgeting. 2. Ask the patient to stretch both hands forward And wait. Not for two seconds. Small irregular movements may start appearing in different fingers, wrist or shoulder. 3. Look for apparently purposeful movements This is a beautiful feature of chorea. An involuntary movement may get converted into touching the face, adjusting the clothes or changing posture. So what looks voluntary may actually have started as chorea. 4. Ask the patient to keep the tongue out The patient may bring it back repeatedly despite trying to maintain it. 5. Test sustained grip Instead of a constant grip, you may feel tightening and loosening. This is motor impersistence. The classical bedside description is the milkmaid grip. Then make the patient walk. A small extra step, shoulder movement, hand movement or sudden change in posture may get mixed into an otherwise normal gait. The important point is that chorea is not one particular movement. It is a pattern of unpredictability. Irregular. Non-rhythmic. Non-repetitive. Moving from one body part to another. And once I am sure that the movement is chorea, only then do I ask the next question: Why does this patient have chorea? Acute or chronic? One side or both? Drug related? Metabolic? Autoimmune? Nice case by Cho, Hallett and colleagues in Movement Disorders Clinical Practice Key Points: - Spinal cord or basal ganglia lesions. - If spinal cord origin, the movements tend to continue during sleep. - An MRI may be helpful if a structural lesion is present. - Physiological studies can be helpful. My take: This case they present turned out to be a 'functional movement disorder' and the movements disappeared with sleep. There was no exposure to a dopamine blocking medication. There was no history of abdominal surgery. The authors remind us to uncover possible 'suggestibility, entrainability, and distractibility.' PRONATOR TERES IN THE THROWING ARM The Pronator has 2 insertions on the inside of the elbow 1) To the Medial Epicondyle through the Common Flexor Tendon 2) To the medial portion of the Ulna (Deep) In 83% of arms, the Median Nerve Runs right between those two heads Due to throwers commonly having hypertrophied and toned up Pronators, this can become an issue with compression of the Median Nerve which runs down the forearm and into the thumb, index, and middle fingers When tight, it can limit Supination and Elbow extension With Pronation and Supination, it is important to test both Range of Motion and Strength in both the Elbow flexed and extended positions as this will affect the muscle lengths of each Trigger points here typically refer deep into the wrist and Anterior Forearm In chronically “tight” Pronators, check the opposing muscle groups (Supinator and Biceps) as you may find they are significantly restricted and essentially forcing the Pronator to “Drive with the E Brake on” with every contraction OR You may see excessive weakness that allows the Pronator to Bully it into living in the Pronated, shortened position This is a non exhaustive list of some quick nuggets in regards to the Pronator Teres as there are a ton of global and external issues that can play a huge factor here

This patient had a >100 cc prostate with severe LUTS and bladder outlet obstruction, and a clear desire to be free from his urinary symptoms and improve his quality of life.
