High-velocity bullets cause a large volume of damage due to the distribution of extreme energy throughout the wound. The degree of destruction can also be increased by the bullet hitting a bone, changing direction, somersaulting, and the shattering effect of the broken bone.

Medically, these wounds are particularly devastating because the fragments tear through soft tissues, organs, and blood vessels, making surgical removal extremely difficult. Unlike a standard bullet, which may leave a single wound tract, fragmentation creates multiple injury paths, increasing the risk of hemorrhage, infection, and multi-organ damage. The irregular shape of the fragments can also cause extensive cavitation, leading to secondary tissue destruction beyond the immediate impact site. Also, metal fragments left in the body may lead to chronic inflammation, lead poisoning (if lead-based), or delayed foreign body reactions. From a trauma care perspective, treating these injuries requires advanced imaging to locate fragments and often multiple surgeries to stabilize and prevent complications like sepsis or compartment syndrome.
