🔬 Why Perfect Visualization of Stents and Calcified Plaques Requires the Highest Spatial, Temporal and Contrast Resolution: The Case for Ultra-High-Resolution with Photon Counting CT Image: a close look at a RCA stent with very mild intimal hyperplasia. Seeing is everything in coronary imaging. Yet when it comes to stents and heavily calcified plaques, even the best conventional CT systems hit their physical limits — blooming, partial volume effects, and metal artifacts blur what truly matters. That’s where Ultra-High-Resolution Photon Counting CT (PCCT) changes the game. 🧠 Why it matters: Conventional CT often overestimates stenosis in calcified vessels and hides in-stent lumen due to blooming and noise. Can you imagine this RCA was completely occluded? We recently treated a young patient with a chronic total occlusion (CTO) of the RCA. Given the poor antegrade options and well-developed collaterals, we opted for a retrograde approach. After successful crossing, lesion preparation was followed by a "leave-nothing-behind" strategy using four drug-coated balloons (DCBs)—no stents implanted. The image you can see below is a follow up performed 3 months after the CTO PCI. Why this approach? Preserves native vessel function and vasomotion Avoids permanent implants and associated long-term risks Reduces need for prolonged DAPT Makes future interventions easier, especially valuable in younger patients. Sometimes IVUS guided direct stent still leads to distal embolus. Went after it with a little pokey pokey with my wire and broke it up in the apex.
