After relapsing on the only drug in development for my rare disease, my doctors told me ‘We’ve tried everything, there’s nothing more that we can do.’ To make things worse, I didn’t have a billion dollars and 10 years to develop a new drug from scratch. My only hope was to find a drug that was already FDA-approved that I could repurpose for my disease. It’s been over 9.5 years that I’ve been in remission and we have been able to identify and advance numerous treatments for diseases they weren’t initially intended for. Hey, I'm Dryka Brenda, and I'm facing a difficult battle against a rare genetic disease called Familial Adenomatous Polyposis (FAP). It's caused by a mutation in the APC gene, which my father and I both have, and it causes the growth of many polyps throughout the digestive system. FAP doesn't just affect the large intestine; it can also affect the stomach, duodenum, small intestine, and even other organs like the thyroid, liver, pancreas, adrenal glands, bones, and skin. It's a silent disease that spreads internally and, if left untreated, almost inevitably leads to cancer. We only discovered this mutation in 2023 when my father was diagnosed with two tumors in his intestine, one in the rectum and the other in the colon (synchronous tumors). After that, he and I underwent surgeries to remove our large intestines, but unfortunately, this disease is progressive and is now affecting our stomach and duodenum. This mutation has already caused a lot of pain in our family. My grandmother passed away from stomach cancer, my brother from leukemia, and now my father and I are in this fight. But there is hope: an experimental treatment and specialized follow-up at the Mayo Clinic in the United States (a world reference in rare and genetic diseases). Both my father and I will be treated there, and my mother will go along as our caregiver. We have already created a crowdfunding campaign to raise the necessary funds, but it is not enough, and we need to raise this amount by January 22nd, otherwise we will not be able to undergo the treatment. The Token fees will help me pay all the costs: passports, visas, tickets, food, accommodation, medical translations and unexpected costs that arise, and mainly, the treatment It's difficult to ask for help, but we are clinging to the chance to live with quality of life and the chance to control this disease. It's not uncommon to find 15, or even 50, different organizations dedicated to a single rare disease. This is without a doubt holding the rare disease community back. The reality is that compared to more common diseases, rare diseases already face significant hurdles in accessing resources. This scarcity is amplified by the fragmentation of groups. Each organization, operating in isolation, struggles to gather sufficient patient data and biospecimens, which are crucial for advancing research and treatment options. Imagine the potential if these groups could unite their efforts. By combining resources, sharing data, and collaborating on research, we could accelerate progress in ways that are currently out of reach. Thanks Ashish Ashish M. Kamat, MD, MBBS. Your book entitled, "Bladder Cancer: A Practical Guide" says it all. Together we face patients on a day-to-day level in which we have to make decisions in the best interest of the patient with nothing else considered. I was so grateful for you to have shared with me your views on whether CIS and papillary from a clinical perspective was the same disease and also how you treat patients who have failed all BCG and have papillary disease and face losing their bladder to a life-changing operation of cystectomy. I was blown away at this year's #AUA2026 when a researcher presented a slide showing that the morbidity and mortality of a cystectomy exceeds even that of a Whipple in which the pancreas, gallbladder portion of the stomach and spleen are all removed. That blew my mind especially since there is an option to save the bladder. I am posting the segment of our long conversation relating to your views of whether CIS and high grade papillary are treated clinically as the same disease. I was glad to hear the next day at the FDA workshop that your fellow expert colleagues completely agreed with this sentiment that you expressed in our conversation.
