Zack's expression in this moment is so despairing. For all Zack knows, Cloud's still in a coma with no signs of waking up, and Zack's bleeding out, dying. There's gonna be no one to take care of Cloud; he'll waste away, get mauled by monsters, or worse, be recaptured by Shinra

Then he sees painstakingly familiar blond hair engulf his vision and feels such an overwhelming sense of joy. There's a wince of pain at first as he tries to get up, (is he hallucinating?) but he can't with all his injuries. He HAS to believe that Cloud is finally awake.

The sudden happiness and relief pumps adrenaline into him, enough to reach for Buster and pass it over to Cloud. Who 𝘢𝘤𝘤𝘦𝘱𝘵𝘴. He might be dying, but at least Cloud is 𝘢𝘸𝘢𝘬𝘦. He can't help the smile. He says everything he can to make sure that Cloud will keep on living

And Zack dies smiling, without regrets, knowing that he was able to save one person: Cloud.
