what is the clinical history? we published a similar case case [Rev Cardiovasc Med. 2016;17(3/4):85-99] where an aortic root abscess caused extrinsic compression of the LM, LAD, and LCx

I would be concerned for Aortic dissection with LM extension. Otherwise SCAD, Spasm, less likely compression.
