A high percentage of patients are unable to undergo a valve-in-valve TAVI procedure due to the high associated risks of sinus sequestration, coronary occlusion, or prevention of future coronary reaccess. The Splitter device, currently under testing, performs leaflet modification by creating an intentional U-shaped excision of the valve’s cusp tissue by a steerable catheter with an electrocutting wire loop running inside a cutting head that mimics alligator jaws.

It's always fascinating to watch the outflow valve (OFV) close during takeoff. This marks the beginning of the aircraft’s pre-pressurisation cycle. While it does provide a smoother transition to pressurised flight and improves passenger comfort, its main purpose goes beyond that. The primary reason for closing the outflow valve at takeoff is to prevent a slight pressure bump that occurs during aircraft rotation. At high angles of attack, an air cushion effect forms over the aft outflow valve, which can cause a momentary reverse airflow - potentially pushing air back into the cabin through the OFV. This pre-pressurization prevents FOD from being blown into the cabin, and this is also the reason the airplane lands with a slightly pressurized cabin. #FunFact

A Medtronic Evolut valve embolized during placement. During rapid pacing, a beat occurred and ejected a partially deployed valve. It migrated to the innominate. We retrieved it via sternotomy. A transverse aortotomy was made and the valve easily removed. A new EDWARDS magna surgical valve was implanted and the patient did well. The Saturn valve (InnovHeart) is a novel TMVR bioprosthesis that has a central valve mechanically connected to an annular ring. It features a low-profile design with a small footprint in the left ventricle. The annular structure and the anterior connecting arm immobilize the anterior mitral leaflet, which prevents systolic anterior motion. This is fascinating video concept comes from a study performed to assess the procedural performance and long-term healing profile of this TMVR system in healthy preclinical models.
