I want to share a story and you can make your own conclusions. I just operated on a patient with breast cancer who had tissue expanders placed a couple weeks ago. While recovering at home, she had a seizure and went to the ER. They ordered a brain MRI but here’s the problem: MRIs and tissue expanders don’t mix. Why? Most expanders have metal in the ports. During an MRI, the magnetic force can pull or heat that metal, damaging the expander—or worse, harming the patient. In her case, the expanders tore under the pressure of the scan. She even told them she had expanders in, and felt pain during the MRI. Unfortunately, she’ll need ongoing brain MRIs, so today I removed her expanders so she can safely continue treatment. But the bigger issue here is how a system so large can let something like this happen, even when the patient spoke up. Too often we’re told, “It’s nobody’s fault, it’s the system’s fault.” But patients deserve better. I’m just grateful she wasn’t harmed more, and that we could take care of her today.

Dynamic ultrasound can reveal knee instability that static imaging may miss. 🔍🦵 During a combined maneuver (valgus stress + anterior drawer + tibial external rotation), ultrasound may show anterior translation of the medial tibial plateau followed by a sudden posterior snap of the posterior horn of the medial meniscus over the posterior tibial rim. Interestingly, this can occur without a visible meniscocapsular tear on ultrasound or MRI, suggesting a dynamic functional instability of the posteromedial meniscus. 💡 Clinical pearl This finding may represent an early stage of anteromedial rotatory instability (AMRI) and could precede the development of ramp lesions or longitudinal meniscal tears. 🧠 How I figured out a way to explore it • Left hand: performs the anterior drawer • Right hand: holds the probe on the posteromedial knee and applies valgus stress • Body: stabilizes the foot and applies the tibial external rotation Yes… it takes a few limbs. Today on CBC Metro Morning, former Ontario Premier Bob Rae, in discussing the recent Toronto Metropolitan University antisemitism report by The Honourable Mary Lou Benotto, attempted to draw an equivalence between the torrent of antisemitism plaguing our post-secondary campuses and alleged "hate" that is directed at Palestinian students. This reflexive pairing must stop. When a report documents antisemitism, address antisemitism directly, without immediately invoking Palestinian students as a counterweight.
