Done that beyond the Break Wall on Lake Superior when young and really stupid.
Why isn’t a securing rope and harness system used also? Line thrown from ship to pilot boat and D ring used to secure to pilot. In the event of a slip at least you’re not plunged in and the deck team can drag you up?
