Excited to share TERRA, a tissue world model ๐Ÿงฌ Over ~1.5 years we ran a large data-generation + modelling effort to build a world model for human tissues, pretrained on 112M cells from spatial transcriptomics (mostly Xenium 5000-plex + public data). It's built on one of the largest human spatial transcriptomics corpora assembled to date, spanning 20 tissues across development, health and 26 disease conditions, ~two-thirds newly generated in-house. Why a "world model" for tissue? Images have universal representations (ViT/DINOv3), so do proteins (ESM, Alex Rives) and pathology (UNI, Faisal Mahmood). We've worked hard to build something similar for human tissue: one model that captures its multi-scale logic, genes โ†’ cells โ†’ their native microenvironments. Like the JEPA approach Yann LeCun has championed, TERRA learns by prediction in embedding space, but for human tissue. How it works: it tokenises each cell together with its nearest neighbours into one sequence while keeping every gene's identity, then masks part of a neighbourhood and predicts the representation of the hidden part, not raw noisy counts. From one backbone it reads out three scales, gene embeddings (what a gene is doing in a cell and its niche), cell embeddings (cell type and state) and neighbourhood embeddings (the niche), and because it keeps gene-level resolution it can knock a gene out in silico and predict the response. Applied entirely zero-shot, TERRA maps and perturbs human tissue across unseen organs, diseases and technologies, outperforming existing spatial approaches. Three take-homes: 1๏ธโƒฃ One model, any tissue. A single pretrained backbone provides tissue representations zero-shot, handling genes, cells and niches across organs and platforms, off the shelf. 2๏ธโƒฃ New biology, development to clinic. We built a new spatial atlas of the developing human pancreas and found an islet-associated capillary state that looks like a precursor of mature islet vasculature. In kidney, TERRA's in silico knockouts predicted the tissue-injury programme from cancer immunotherapy (checkpoint blockade), confirmed in treated kidneys, detected in blood, and linked to declining kidney function. 3๏ธโƒฃ A grammar of tissue architecture. By coupling each cell's state to its niche, TERRA defines recurring cross-organ "archetypes" of macrophage neighbourhoods, including a tumour-boundary niche that tracks poor survival in kidney cancer. TERRA is already in use: it powered our recent skin atlas of hidden immune-memory niches ( with more studies coming soon. The Department of Justice just proved my point. Last week a supermarket pharmacy chain โ€” Ahold Delhaize, the parent of Giant, Stop & Shop, Hannaford, and Food Lion โ€” agreed to pay $40 million to settle False Claims Act allegations. The charge: they reported inflated "usual and customary" prices to Medicare Part D, Medicaid, and TRICARE, and in doing so made the government overpay on prescription after prescription. $32.9 million of that goes back to the feds, the rest to state Medicaid programs. Stay with me, because this sounds backwards until you understand the machinery. "Usual and customary" is just insurance jargon for your normal, everyday cash price โ€” what a walk-in pays with no card, no plan, no middleman. That's the same price I've been talking about for days. And here's the part nobody outside the counter understands: if you accept insurance, that price MUST be high. It's not greed. It's survival. Insurers reimburse pharmacies at the LOWER of two numbers โ€” your usual and customary, or their contract rate. So the second you post honest, low, transparent prices, you've capped your own ceiling. Every claim defaults to your low number. Meanwhile the reimbursement model itself is garbage: pharmacies eat zero-dollar and below-cost reimbursements every single day. The only way you stay open is the occasional script that pays well enough to cover the pile of losses. List your real prices and you kill the high payers โ€” but you're still contractually bound to absorb the losses. So transparency, under an insurance contract, means you ONLY take losses. That's why discount cards exist. They're a separate program, not your U&C โ€” a workaround. The catch is they charge the pharmacy fees that wipe out most of whatever profit was left. Which brings us right back to this case. Ahold did exactly what Walgreens and Kroger tried: list a high U&C for insurance, then run a separate "prescription savings program" with the real, lower prices on the side. DOJ's position is simple โ€” that savings-program price WAS the usual and customary, and burying it is fraud. Walgreens paid $100 million. Kroger settled for $17 million. It's a pattern, and everyone who tries it gets shut down. Here's the kicker. This was a whistleblower case. A single pharmacist filed under the False Claims Act's qui tam provision โ€” a Civil-War-era law that lets insiders sue on the government's behalf and keep a share of the recovery โ€” and walked away with $6,083,587. So if you work at Amazon Pharmacy, you may want to take a hard look at that U&C. This is the whole reason we fired insurance. We don't accept it. We aren't bound by anyone's contract rate, we don't report a U&C ceiling to a PBM, and we don't bleed our prices through discount-card fees. It's the only honest way left to price a prescription lower than the ridiculous insurance prices.

Ali Ghodsi never wanted to be CEO. In 2015, he was interviewing for a professor job at Berkeley when the Databricks board handed him the interim title. Revenue that year was $1.5M. This episode with Ali Ghodsi will go down as one of my favorites. 10 things I took away: 1. Focus the entire company, orders of magnitude of attention, on its single biggest bottleneck. Like a laser, almost to an extreme. The cycle is 1-3 years, not weeks. If your focus changes weekly, then youโ€™re just in firefighting mode. 2. There is nothing worse than a conflict-averse CEO. They are wonderful people, but they are in the wrong job. Conflict is the gym for a CEO: nobody likes it, but everyone has to go. 3. Study your enemy carefully, understand their weaknesses and apply your strengths to those weaknesses. Snowflake had 2x his revenue. He didnโ€™t copy them. He found three weaknesses (proprietary, no AI, expensive) and hammered them account by account for four years. Watch the competition, never follow it. 4. The concept of a Lakehouse was ridiculed internally and online. No one wanted to market with this new term. So he made the whole company religious about it anyway, killed the ads that converted better without the word, and put it in the sales comp plan. It worked. All hands on deck, no exceptions. 5. Be willing to take a step back for a much bigger vision, even when the company is already succeeding. At multiple hundreds of millions in ARR, he was unhappy, because the vision he pitched investors wasnโ€™t the company he was running. So he took one step back to go ten forward. 6. On the flat org, player-coach model that a lot of people have talked about this year: โ€œitโ€™s BS.โ€ Separate how the company thinks (AI, ontology) from how the humans get managed (they are after all, still humans). His staff meets 3x a week. I asked if it could just be coordinated in a Google doc. His answer: do you meet your wife and kids, or coordinate that in a Google doc? 7. His test for a sales leader: can they build the car, or just drive it? Ron Gabrisko, the Databricks CRO, had seen $0โ†’50M and $50โ†’100M+, and hadnโ€™t changed jobs in 10 years prior to joining. Now he has been the CRO for over a decade. He built and drove the car the whole way. That almost never happens. 8. Hire execs ahead of the curve because by the time you need them, itโ€™s too late. A real search takes 6-12 months. The extra time helps you increase false negatives and decrease false positives. Do an insane number of backdoor references because 80% of โ€˜front doorโ€™ references are bs. 9. The best salespeople are not super technical, so stop trying to force them to be. Square peg, round hole. The best win with professional aggression, high EQ, and mapping the real power base (how decisions get made high up in an organization), not technical depth. 10. Yes, your best AEs will annoy people. One of the first at Databricks got a meeting nobody could get, but got banned from the customerโ€™s building for it. He told Ali, โ€œwhat are you complaining about? I got the meeting.โ€ Professionally aggressive is the bar. One bottleneck, zero wussing out. He reminds me of Elon Musk that way.
