Witness D previously testified that an operation he had been invited to participate in ended with a suspect dying after being tortured. It later emerged that Mkhwanazi was called to the warehouse where the man had died. Despite that, he saw no need to inform them or request a meeting whilst they were all in KZN about his plan to disband the PKTT.
