Out-of-pocket payments remain the dominant source of health care expenditure for most of our people, translating into higher costs and lower standards of accountability for providers. When the Federal Ministry of Health and Social Welfare Federal Ministry of Health, NIGERIA signed the Renewed Hope Ministerial Performance Monitoring Bond with its detailed key performance indicators, we set ambitious 2030 targets that are now within reach, one of which is national health insurance coverage. Under the leadership of Mr President, all macroeconomic reform and cross-sectoral policy direction are driven by one objective: ensuring that every Nigerian can actualize their aspirations while accessing affordable, quality care. This people-centred approach defines the comprehensive reforms now reshaping our health sector. As we modernize infrastructure across primary, secondary, and tertiary facilities in all local government areas, our record-breaking progress in national health insurance enrollment is aligning demand with Nigeriaโ€™s rapidly expanding supply of human resources and infrastructure. Nearly 120,000 health workers have been trained since 2023, and 2,500 doctors, nurses, midwives, and community health extension workers have been recruited to strengthen frontline services. Over the past 16 months, 4,000 health personnel have also been added in Federal Tertiary Hospitals to close workforce gaps. In 2024, Nigeria broke its enrollment record with more than 2.4m new people insured, bringing total coverage to about 20m Nigerians. With new policy tools in motion, we are on course to reach 44m by 2030. This is the surest path to reducing out-of-pocket expenditure, which still represents about 70% of total health spending. Social health insurance schemes now account for 90% of all enrollments nationwide. A major constraint in care quality has been the low capitation fee for enrollees. For years, the capitation stood at โ‚ฆ750 per person. We have doubled it to โ‚ฆ1,450 to ensure providers are properly equipped to deliver consistent, high-quality care. Fee-for-service rates have also been increased by 380%, based on actuarial evidence aligning cost-reflective rates with quality requirements. The Federal Government Government of Nigeria has also introduced the One Hour Referral Authorization Code, issued by the insuring entity to the provider to confirm referral approval. By shortening this process to 1 hour, we are eliminating unnecessary delays and ensuring that patients move quickly from primary to specialist care. To strengthen accountability, I have directed the National Health Insurance Authority NHIA NIGERIA to commence covert mystery shopping to monitor providers and ensure that enrollees are not denied treatment. Undercover monitors are already observing processes in facilities nationwide to ensure that insured Nigerians receive timely and dignified care. The evidence is clear. From fewer than 10m hospital visits in all of 2023, more than 46m visits were recorded by the second quarter of 2025. Reforms in the Basic Health Care Provision Fund Basic Health Care Provision Fund and the NHIA are delivering measurable gains in patient confidence and service utilization. As enrollment expands, benefits multiply. Wider adoption of health insurance enhances quality, strengthens accountability, and protects households from financial hardship. These achievements reflect President Tinubuโ€™s vision of a health system that serves all Nigerians, regardless of income or status. We will continue to modernize infrastructure, strengthen the workforce, and sustain reforms until every Nigerian is covered and cared for. I visited the Thessalon Hospital and heard from health care workers stretched to the breaking point: people getting sicker without family doctors, emergency rooms overwhelmed, staff burning out. These challenges are hitting northern and rural communities hardestโ€”but theyโ€™re spreading across Ontario. When governments fail to invest in local capacity and download responsibility onto hospitals already stretched thin, patients pay the price. Doug Ford is accelerating the privatization of our health care by diverting billions to for-profit clinics. Funding private clinics diverts money from already stressed public hospitals. Heโ€™s paying private clinics more than double what hospitals get for the same surgeriesโ€”less value for every public dollar. Funding to for-profit clinics increased over 200% while public hospitals faced real cuts. Heโ€™s moving 50% of surgeries to private facilities. In eight years under Doug Ford, access to health care has become worse. Not better. And hereโ€™s whatโ€™s most important: Universal public health care is a Canadian value we cannot afford to lose. This is yet another instance of private health insurers going back on their commitments. Many middle-class citizens are increasingly turning to private health insurance providers as a critical lifeline for themselves and their families. These plans boast of a robust coverage and hassle-free claims, while charging customers exorbitant premiums. But when insurers fail to honour genuine claims, it leaves citizens vulnerable and puts a Question mark on the safety net of health insurance. As per information given by the IBAI (Insurance Brokers Association of India), the best performer in the Claims Paid Ratio (The total number of claims paid divided by the total number of Claims available for processing) was New India (Public Sector General Insurers) with 95.04%, followed by Aditya Birla Health (Standalone Health Insurer) at 94.52%. Other Large General Insurers such as IFFCO Tokio, Bajaj Allianz, SBI General have claims paid ratio of 91.70%, 90.29% and 88.86%. Other Standalone Health Insurers such as Niva Bupa, Manipal Cigna and Care Health have around 88% claims paid ratio. However, if one takes a look at the Claims Paid Ratio on the Amount of Claims (The Total amount of Claims paid divided by the Total amount of Claims available for processing), New India, Oriental Insurance, National Insurance - all public sector - have impressive ratio of 98.74%, 97.35% and 87.95%. The same for Standalone health insurers Aditya Birla Health, Niva Bupa, Manipal Cigna, Care Health and Star Health are all less than 71%. Similarly, the Health Claims Paid Ratio based on the amount for Small Private Sector General Insurers such as Liberty General, Acko, Zurich Kotak & Royal Sundaram reflect poorly. This is not to target anyone, but highlighting the need for better regulation and grievance redressal for the customers. After all, in the 2022-23 period, the health insurance sector issued 2.26 crore health insurance policies. And these policies provide coverage to an impressive 55 crore individuals. But not even one private insurance provider reported payment of more than 90% of the amount claimed and only 3 paid more than 80% of the amount claimed in 2022-23. During a Zero Hour discussion on December 4, 2024, I had raised the need for fair claim settlements for the citizens of the country relying on health insurance.
