🚨 Relieved of Command Cmdr. James Koffi was relieved of command of the USS Truxtun (DDG-103), an Arleigh Burke-class guided-missile destroyer, by Rear Adm. Carlos Sardiello, Commander of U.S. Naval Forces Southern Command/U.S. 4th Fleet. The reason cited was a "loss of confidence" in Koffi's ability to command, directly following a collision on February 11, 2026, during a replenishment-at-sea (underway refueling) operation in the Caribbean Sea, within the U.S. Southern Command area of responsibility. In the incident, the Truxtun collided with the USNS Supply (T-AOE-6), a Supply-class fast combat support ship (often referred to as a fleet oiler). Two sailors sustained minor injuries and were reported in stable condition. Both ships remained seaworthy and continued operations afterward. The Truxtun is currently in port in Ponce, Puerto Rico, for assessment. Video footage from the Supply shows the Truxtun turning to starboard and striking the Supply amidships during the maneuver. The cause of the collision remains under investigation. Relieving a commanding officer after such an incident is standard Navy practice to ensure accountability, especially in cases involving collisions, groundings, or other mishaps at sea.Cmdr. Koffi had assumed command of the Truxtun on January 9, 2025. He has been temporarily reassigned to Commander, Naval Surface Group Middle Atlantic. Cmdr. Taylor Auclair (previously with U.S. Fleet Forces Command) has taken over as the ship's commanding officer. This event occurred amid a broader U.S. naval presence in the Caribbean, following the Truxtun's departure from Norfolk on February 3, 2026 (after a brief delay for equipment repairs).
