Not all mentally unstable people present as “sad” its a manic state of instability. Happy one minute. Depressed the next. Calm then anxious and paranoid. People who are severely mentally ill or in crisis fluctuate its not just one thing. Sometimes people present as almost euphorically happy right before the “crash” Hes been showing “signs” for a while if you know what the signs and behaviours are.
