4 A.M. STEMI call. 45-year-old male with acute anterior STEMI activated through the STEMI pathway. Coronary angiography showed complete thrombotic occlusion of the LAD. Initial ballooning restored flow, and a drug-eluting stent was deployed successfully… or so we thought. Seconds later came one of the true nightmares in interventional cardiology: The stent was physically delivered. Deployment was confirmed. But fluoroscopy showed… no stent in the lesion. The LAD still looked unstented. Immediate full fluoroscopic search of the patient’s body was performed. No lost stent was seen anywhere. The table? Negative. The sheath? Negative. The guiding system inside the body? Negative. Unfortunately, stent boost was unavailable at that moment, and IVUS was not accessible. Multiple vigorous flushes of the guiding catheter failed to reveal the missing stent. At that point, the concern escalated dramatically — embolized stent? Lost in the system? Somewhere unseen? Decision was made to retrieve the entire system and physically inspect everything outside the patient. Still nothing. Finally, as a last attempt, the guiding catheter was carefully cut in small external segments. And there it was. The entire undeployed stent was trapped inside the OUTSIDE portion of the guiding catheter — beyond the fluoroscopy field — firmly stuck despite repeated flushing attempts. Like something like NET would be considered a breakout. It’s up more than 4%. Its ADR is 5%, so this would be less than its ADR. But you buy something like Citibank. Citigroup, when this thing is up 4%. A 4% move for something like this would be very rare. It’s almost two times ADR. Okay, like you can’t put a hard rule like oh, 4%, because it’s gonna be very different. You’re gonna have very different outcomes on different types of stocks. Like on a slow-moving stock, which you should never trade to begin with. You buy a 4% breakout, you’re chasing big time. I don’t like the 4% rule. And EP too, like EP doesn’t necessarily have to be 10% all the time. Sometimes it can be less. But again, it depends on the stock. If it’s a slower-moving like large or mega cap stock, an EP can be less than 10% gap up, like something like JD. This was like a hybrid of EP and breakout back in August, which I bought size. Cancer Update We’ve made it back to Switzerland. Last year’s trip ended up with me in hospital for 2 weeks when we got back and shortly after discharge was given a few months (basically taking me up to Christmas) to live. BUT I pulled through albeit now only 8 stone and very limited to what I could eat. So we booked Switzerland THEN the stent started blocking and I ended up in hospital having a new stent fitted and looking like we wouldn’t make it because I only got out on Friday - but we did. We both REALLY need this break. I absolutely love my beautiful wife Kay and adore being in our 2nd home with her ❤️ 🇨🇭⛴️. X PS.
