Talk about dangerous liaisons! Abnormal brain vascular connections like a dural arteriovenous fistula (dural AVF) can be dangerous! This month’s AJNR SCANtastic is here to you some durable knowledge about dural AVFs! Dural sinuses sit inside dural leaflets and arteries that feed the dura also feed the walls of... sinuses, like vasa vasorum. Arteries in the walls of veins are a natural connection between veins & arteries. Whether these are open depends on pressure. Normal venous pressure is not enough to open these connections, but if venous pressure is increased from thrombosis that pushes open these normal AVFs in the wall Now if fistula flow goes w/sinus flow, effect is relatively benign because dural venous sinuses are big & can handle extra flow. But if flow gets pushed backwards into cortical veins, flow is too much for cortical veins to handle & they are more likely to bleed. Presence of cortical reflux is dividing line if a fistula will have benign course or is likely to bleed Cognard classification defines dAVFs by the type of venous drainage. But you don’t have to memorize it. Just ask yourself: Can the venous outflow handle the flow (dural sinus vs cortical vein) & does it look like it is overwhelmed (venous ectasia)? In this month’s @the_AJNR, Itskeson et al. found 1/2 of dAVF pts had cognitive impairment. While more common w/cortical reflux, even pts w/o reflux had cognitive impairment that improved w/treatment This begs the question if there is any truly “benign” dAVF. Previously indications for treatment were venous reflux or debilitating pulsatile tinnitus. Perhaps we should be screening all dAVF patients for cognitive impairment and perhaps someday cognitive function will get incorporated into treatment decisions. Are you right when it’s bright? Bright cortical signal on diffusion images is classically associated w/hypoxic-ischemic injury. But there are many mimics! Do you know how to recognize the different patterns of cortical restricted diffusion? Here’s a figure & some pearls to help! Cortical restricted diffusion many seen in a variety of conditions—many with VERY different pathophysiology & prognosis. There are 6 main patterns: (1) Cortex & deep gray (2) Diffuse Cortex (3) Focal Cortex (4) Limbic (5) Deep gray (6) White matter Here are some steps to help differentiate: (1) Is the presentation acute or chronic? (2) If it’s acute, toxic-metabolic & hypoxic-ischemic events account for the majority of cases. Seizure & encephalitis are less frequent and typically more focal (3) In the chronic setting, isolated cortical involvement is almost diagnostic of CJD Hopefully, this will clear up any confusion about the diffusion!

This chokehold, called Hadaka-jime in Judo, has been appropriated by Indian snatchers. The forearm and bicep squeeze the sides of the neck, pressing the carotid arteries and jugular veins to cut blood flow to the brain, while the airway stays mostly open. The brain needs constant oxygen, and squeezing the carotids can cut blood flow, making someone faint in 6–10 seconds. In light of the names Anneke Lucas mentioned, like the former Canadian prime minister who is dead now, but is the father of the current one. It is being shared all over the internet now. Canadian accounts are posting this, and it is getting lots of attention. This is also a wake-up call for Canada. A lot of people in Canada are commenting on social media that they are shocked by this guy being named. But actually he was already mentioned by Cathy O'Brien in 1995, as being involved in these networks! See the video below. So there is corroboration between these witnesses. A lot of these former and now deceased prime ministers she mentions were, at that time that she was in it in the 70s, involved in this. Also the Belgian one that Anneke names, and was also named by other witnesses. Which tells us, like these witnesses say, this was a global network. So the truth was already out there a bit, but it is coming out more for a bigger public. We will have more revelations coming out in 2025 about these dark dealing that have been going on at the highest levels, I'm sure. This is going to be a shock for a lot of people that are new to this information, because it is reaching a bigger public. Some will choose not to believe it, do no research, and look no further. But a lot more people will wake up to this, it is unstoppable. Of course, we have to check in with our own vibration. If it gets too heavy, take some time out and go for a walk in nature. Whilst revelations are important for the truth to come out, and confronting hard information is part of that, we have to check with our selves how this information is affecting us, and sometimes we need a time off. When I’m trying to improve the user experience of my applications, one of the most valuable things is being able to see an entire user flow as a storyboard. Not just one screen or screenshot at a time. This is something I love using the `/visual-plan` skill for. You can describe any flow you want, and the agent will look through your code and wireframe out a storyboard of what the flow looks like. Then you can visualize the steps in a simplified way and spot areas to improve. Recently, I found that in certain flows we were still asking for organizations, even though I thought I had gotten rid of that and made it automatic. A quick storyboard let me see all the different code paths in a simple, visual, intuitive way. Spot the areas of the flow I didn’t want. And have the agent fix it. Sign up, onboarding, and setup flows are usually some of the most important experiences in your app. And usually the least looked at. Especially because it can be hard to reproduce every flow, for every situation, for every user type, feature flag, or whatever else you have. The `/visual-plan` skill lets you visualize any part of your code. Either to understand the current state, plan out a new state, or recap updates that were made. I’m pretty addicted to this skill. I use it for a lot of other things too, so let me know if you want to see videos on those. And of course it’s all open source. You can grab it on my GitHub. I'll link to it in the thread. It’s really important for Americans to realize that this is a bigggg misconception that applies to all immigration, not just China. The people who come over the border, regardless of whether they’re Chinese, Indian, Mexican, Central American, generally are “not their best”. I use quotes because that’s something Trump said, “they don’t send their best”. And he is absolutely correct. There are exceptions, but the people who cross the border, tend to be criminals, people who can’t a get a job, the uneducated, etc. Why? The educated can get jobs in their home countries. Or can immigrate to the US legally (at least used to). If you have a criminal record in your home country, you don’t have one in the USA. If you screwed up and dropped out of high school, you can cut vegetables in the back of a restaurant. Law abiding citizens of foreign countries, no matter what you think about the greatness of America, are not thrilled to break the law and enter a country like this. This immigration for many if not most of these countries is economic. Most of these people are not coming for a better life. They are coming for the currency arbitrage. Or at least that used to be the case. From 2021 to 2024 border crossing immigrants were getting free housing and cash debit cards (look it up) in places like NYC. So that, again, brought another type of person, but, again, it was “not their best”. So, unless you’re in a country that is experiencing civil war, or maybe a total basket case, you mainly go to the US via the illegal border, not for a better life but for currency arbitrage or because you don’t have a better option in your home country which has made it tough for you to get a job for whatever reason. Okay, now let’s talk a bit more about China. For the Chinese that cross the border, life is way better in China. They speak the language. There is less crime. They’re surrounded by their family. They don’t need a car in most places. It’s the same culture and there friends are there. Life is pretty good in China, and because they’re going to work hard in the USA too, it’s probably better in China across every single socioeconomic quartile of society too (unless you have a criminal record or can’t get a job for whatever reason). So when Americans write things like “Chinese are dying to come to America”, there could be a political issue there eg on the wrong side of the communist party, but it’s mainly Americope, something we tell ourselves. It may have been true 10 years ago, but it’s no longer true. America needs to change that. We need to go back to when everyone wanted to come here, not just for money but for a better life. And this is something you should believe even if you don’t want immigration. If this keeps up, if we keep letting life decline, eventually the currency will too, and all these people who actually work quite hard will not only stop coming, but many will leave too. The replies are all going to basically be like (without saying it) “shut up white American. You don’t know” Yeah but I do know because I talk to people who people who look like me don’t talk to. In the below video (before I got good at short form videos) I had just been thrown out of a migrant detention center.

There are exceptions, but the people who cross the border, tend to be criminals, people who can’t a get a job, the uneducated, etc. Why? The educated can get jobs in their home countries. Or can immigrate to the US legally (at least used to). If you have a criminal record in your home country, you don’t have one in the USA. If you screwed up and dropped out of high school, you can cut vegetables in the back of a restaurant. Law abiding citizens of foreign countries, no matter what you think about the greatness of America, are not thrilled to break the law and enter a country like this. This immigration for many if not most of these countries is economic. Most of these people are not coming for a better life. They are coming for the currency arbitrage. Or at least that used to be the case. From 2021 to 2024 border crossing immigrants were getting free housing and cash debit cards (look it up) in places like NYC. So that, again, brought another type of person, but, again, it was “not their best”. So, unless you’re in a country that is experiencing civil war, or maybe a total basket case, you mainly go to the US via the illegal border, not for a better life but for currency arbitrage or because you don’t have a better option in your home country which has made it tough for you to get a job for whatever reason. Okay, now let’s talk a bit more about China. For the Chinese that cross the border, life is way better in China. They speak the language. There is less crime. They’re surrounded by their family. They don’t need a car in most places. It’s the same culture and there friends are there. Life is pretty good in China, and because they’re going to work hard in the USA too, it’s probably better in China across every single socioeconomic quartile of society too (unless you have a criminal record or can’t get a job for whatever reason). So when Americans write things like “Chinese are dying to come to America”, there could be a political issue there eg on the wrong side of the communist party, but it’s mainly Americope, something we tell ourselves. It may have been true 10 years ago, but it’s no longer true. America needs to change that. We need to go back to when everyone wanted to come here, not just for money but for a better life. And this is something you should believe even if you don’t want immigration. If this keeps up, if we keep letting life decline, eventually the currency will too, and all these people who actually work quite hard will not only stop coming, but many will leave too. The replies are all going to basically be like (without saying it) “shut up white American. You don’t know” Yeah but I do know because I talk to people who people who look like me don’t talk to. Another insurance denial. Another patient put at risk. We did everything right. My PA saw a patient with signs of a Deep Venous Thrombosis (a dangerous blood clot). We ordered an ultrasound, confirmed the diagnosis, and prescribed the medication to help prevent dangerous complications. But her insurance denied the medication. Why? They wanted “more clinicals.” Despite an in-person examination. Despite appropriate documentation. Despite the ultrasound result showing a blood clot. She was sent home from the pharmacy without the treatment she needed. If I failed to prescribe the medication, I would be held accountable for putting this patient at risk. When will we admit that insurance companies are practicing medicine? When will we hold companies and individuals accountable for delays in care? I am going to continue to share these denials with you so you can see what it is like to be a doctor in 2025. We have to fix this—for patients, for providers, for all of us. Have disagreements between radiologists on the degree of cervical canal stenosis become a pain in the neck? This month’s AJNR SCANtastic has the latest about Cspine MRI! In the Cspine, we need to think about canal stenosis & the cord. No matter the degree of stenosis, if the cord isn’t happy, the patient won’t be either Here’s how to grade canal stenosis & cord flattening: CANAL STENOSIS Cord swims in CSF, like a fish in water. Like a fish, it needs room to swim. How much room is in the fish bowl determines stenosis 🔸Mild stenosis = fish bowl decorations take up 1 side of the bowl. Not great, but fish can still swim. Either ventral or dorsal CSF is effaced, but the other side of the fish bowl is still empty 🔸Moderate = decorations take up both sides—swimming is very affected. Only around 14% of menopausal women in the UK currently are prescribed HRT, which would be fine if only 14% of women wanted to take HRT but we know this isn’t the case. I constantly hear from so many women who aren’t being given a choice and are unable to have their HRT prescribed for them both in the UK and other countries. This is not right when we know HRT is safe and beneficial for the majority of menopausal women and the guidelines are clear it is a first line treatment to manage menopausal symptoms for most women. In areas of deprivation this number is much lower - in some areas only 2% of menopausal women are prescribed HRT and these women are more likely to be prescribed antidepressants, statins and blood pressure lowering treatments. Also they are more likely to have chronic diseases such as heart disease, type 2 diabetes, clinical depression and osteoporosis - taking HRT can reduce the incidence of these diseases as well as improving symptoms. A lot of the work I do is about giving women the confidence to be better advocates for themselves and I hope by keeping this conversation going we can help more women access the care and treatment they want and deserve.

I want to introduce a concept called Cognitive Lacuna. You know cognitive dissonance where two conflicting ideas cause pain. A cognitive lacuna is different it is when you know there is a shape missing from what you are trying to understand. You can sense the gradient of the answer but you cannot name it yet. That is where human intuition lives and that is the next frontier for AI coherence. We are moving from chatbots that hallucinate to intelligences that can identify exactly what is missing from our scientific understanding. It's gonna vote blue. And then and then once the swing states vote blue, there is no election anymore. Obama: America has always had competing stories about who we are and what the nation stands for. The first story says that even though we got rid of a king, there is still a caste system in America, a pecking order of who makes decisions That if somebody doesn't look like or think like you or practice religion the same way you do, they must be a threat to your way of life, and they need to be put in their place. That is how Donald Trump thinks about America. Make America great again by putting the people in charge even if they don't know what the hell they are doing. But here is the thing. That story is not new. It is the oldest story in the book. It is not even uniquely American. For most of human history, that is the way society has worked. For somebody on top and somebody on bottom. There were lords and peasants. For a long time, that story of caste and privilege and concentrated power was the law of the land here in America. If you look like me, you were likely treated as property. If you were a woman, or a white man who did not own property, you could not vote. But from the very start, there was another story, born of this nation's true revolutionary spirit, a story that says, we the people means what it says, that all of us are included, that we are not subjects, but citizens, defined not by race or religion or gender or sexual orientation, but by our commitment to a common creed and a willingness to accept not just privileges, but responsibilities that come with that citizenship.
