I kind of wonder if something similar didn't happen when they designed the Space Shuttles. The hatch opened into the lower level of the Crew Module. With the Shuttle vertical on the pad, the Commander and Pilot and two Mission Specialists who were going to be in the cockpit level entered and immediately turned right to go through the opening between the lower level and cockpit - what was basically the "floor" of the cockpit when the Shuttle was on its wheels. Going through that opening, they and their assistants had to stand on a temporary shield placed over the rear facing cockpit windows and rear facing crew station control panels and be lifted over the second row of seats into the pilot and co-pilot seats in front to be strapped in. (This had to be even worse on the early two-person Columbia missions that had ejection seats in the first row until they were removed many missions after being disabled after STS-4.) It had to be quite the acrobat show on those occasions when the crew had to evacuate on the pad, but I don't ever think I saw video of how they did that! Cockpit video of getting the flight crew into their seats...

Maybe, I think this was their backup plan when the loading platform wouldn't rotate down. I think, I don't know though. Seems like I've seen them get out after they lowered it.
