This just could have been him realizing this is the Peter MJ called out to as Spider-Man left their apartment. He spent an undetermined amount of time hanging out with the two of them in his Spidey suit. As for the handshake, the spell was just to erase the memory of Peter Parker, but what if that doesn’t necessarily include his muscle memory of their handshake? You could argue that it should have, but obviously something involving his muscle memory retained it (that’s just how I justify it, if in fact, it isn’t Ned re-remembering Peter.)

Or something, here I bet is him just putting two and two together realizing Peter is spiderman, not actually remembering Peter before the spell. Though Ned is magic so it isn't impossible
