Why a single finger-stick is not evidence that HIV is not blood-borne This is the first place the argument collapses, and it collapses on probability, not ideology. The per-act risk of HIV infection after a percutaneous needlestick involving HIV-positive blood is on the order of 0.23–0.3%—roughly 1 in 333 to 1 in 435 untreated exposures. That figure comes from prospective occupational-exposure studies of healthcare workers, not from theory. Risk is higher with deep injury, a hollow-bore needle that was in a vein or artery, visible blood on the device, and high source viral load; it is lower with a shallow finger prick that transfers a tiny volume. Willner’s act was a shallow finger-stick with a tiny inoculum. The expected outcome of one such event is no infection. Observing the expected outcome of a low-probability event is not a disproof of the event class. It is the same error as concluding that Russian roulette is safe because one spin didn’t kill you.
