I'm a native Japanese speaker and have lived in ENG speaking countries for 2 decades, but I have to confess that I do this to non-native JP speakers. 日本語で話していて難しい熟語を使いそうになったら英単語で代替してました。Never imagined that this creates confusion. やめます。

This happened to me at my physical last year. I just took that shoe and put it on the other foot and glazed how good the Dr's English was. (I didn't understand a word he said.)

Yep, this is pretty much spot on. Earlier today, I mistook Dr. Aoki (青木) for Dr. Haruki (春木). The kanji are quite easy and distinct, but I wasn't paying close attention.
