Done in RPG Maker VX Ace. The lighting is in real time and the doors and signs are true 3D, with recess levels determined by height maps.

It's all real time 3D with dynamic lighting. You can see doors and stuff are properly recessed.
How the fuck did you do this in rpg maker
A basic 3D dungeon system called FPLE. It's very basic on its own with no effects. I decompiled that then added support for lighting, taller walls, recessed textures, then made a bunch of scripts that add faux 3D dust, mist, looking around, that sort of thing.
