Sounds like a bunch of bureaucrats sitting around, trying to protect their bottom dollar while deciding if they want to give you healthcare or not or if the procedure that your doctor clearly wants you to have is covered under your specific insurance sounds like somebody is practicing medicine without a license

People!! IT IS NOT the insurance companies that decide what benefits you get. That decision is made by your EMPLOYER. The employer tells the insurance company what they can and cannot cover. Every employer that provides benefits has a yearly benefits budget.

It's a lot worse than what you're showing. At 59, 12yrs ago, I had a 5 star insurance company. I had a major stroke. I couldn't talk or walk... even swallow. After spending a week in the hospital, 5 Dr's refused my insurance and sent me a 65K bill.
