Did you know that insurance companies can apply for waivers to sell plans that don’t meet the standards of network adequacy? It’s happening here in Texas and across the country. I recently received notification that an insurance company told the state of Texas that they couldn’t find an ambulatory surgery center willing to be in network with them in Austin. They lied. My surgery center wants to be in network with them, and they know that. They used that lie to justify getting a waiver to sell incomplete coverage to Texans. This isn’t just a policy issue—it’s hurting real patients, like the woman who waited months to be diagnosed with breast cancer because her plan quietly applied for a waiver to sell a policy without real access to mammograms. This is a dishonest abuse of the system. We must demand better.

The nurse in charge of this woman’s care doesn’t seem to be taking her job seriously. The woman suffers from seizures. She was afraid that the nurse would neglect her so she set up her camera on her phone. She then proceeds to have multiple seizures while on camera. The nurse in charge of her was nowhere to be found but luckily another nurse walked by and saw what was going on and stepped in. The Dr let out a sigh in frustration that the woman was left alone like that while being cared for seizures. It’s one thing to be lazy and not like your job but when your job is caring over another life, the consequences need to be brutal, should she be fired and fined for her disregard for patients lives?

There is a beautiful Catholic tradition with that piece of cloth (manutergium) which was used to wipe the oil of chrism from his hands. The mother is to keep this cloth for the rest of her life. Upon her death, she is buried with it in her hands. As tradition says, when she arrives to the gates of heaven, she will present the cloth to show that she raised a priest who shared in Jesus's life and suffering.

What she meant was never that AKB was “better” than KLP, or that she was criticizing KLP, Malaysia, or the people around her there. She explained that she is genuinely grateful for the environment she had in KLP, and at the same time, her experience in Malaysia made her realize how many things in AKB she had previously taken for granted. With KLP, they were essentially building a 48 Group and its idol culture from the ground up in a place where that system and environment were not already established. There were a lot of fun experiences, but naturally there were also difficult things that came with having to create something from scratch. Because she went through that experience, when she returned to AKB, she became more aware of all the things that were already established and prepared for the members there, and it made her feel grateful for the AKB environment as well. She specifically emphasized that this was NOT about saying one place was better than the other. She said that both places were wonderful to her, both were places she loved, and that was what she had wanted to convey. She also mentioned that some Malaysian fans had misunderstood what she meant, and clarified that she never intended for her words to be used to attack anyone. So I hope people can listen to what she is actually saying here rather than judging her based only on a clipped portion of her speech. There are moments in medicine that stay with you. This is one of them. Alma is sharing her experience and her story with you to make a difference. Alma and I are the same age, and we both work in healthcare. In 2021, she knew something wasn’t right. Her abdomen hurt and kept swelling. Her symptoms were addressed, but she was reassured that she would feel better soon. She visited multiple doctors and emergency rooms before finally being diagnosed with metastatic ovarian cancer at age 44. She was also found to carry the BRCA1 mutation. She found a wonderful oncologist and began treatment, but when her cancer progressed on the strongest medicine available to her, her doctor suggested that she go to MDAnderson for an alternate treatment. Alma has medical insurance, but says that when she reached out to MDAnderson, she was told it would cost $16,000 upfront to have her records reviewed and see a doctor for a second opinion. And this is only part of her story. There is so much more she has endured — more than can fit in a single video — and it deserves to be heard. Regardless of where you stand on policy, insurance, or systems, we can all agree that this is not the way insured patients should experience cancer. Her words that keep echoing in my mind: “Listen to your patients. We know when something is wrong.” Something is wrong, America. Let’s agree on that. Thank you for trusting us with your story Alma. 📣 BY THE WAY MEGHAN (NEE) MARKLE IS NOT ASHAMED OF HER HER FIRST NAME ‘RACHEL’. Decided to address this long running weird theory that is not for the people that are not weird. 🙂 I’ve always found it fascinating that deranged royalists think that calling Meghan, The Duchess of Sussex by her first name ‘Rachel’ would upset her and that calling her by that name is like a slur, insult or jab to her. It’s not! Meghan is her middle name used by her parents from birth simply because they preferred it in the end to the first name they gave her. It’s that simple. As shown in the clip below 👇 where Meghan happily volunteers to Tv host Rachel Ray in May 2018 that ‘Rachel’ is actually her first name, she is not ashamed of it and if so she wouldn’t have mentioned this as a fun fact about her. 🙂 If not interrupted Meghan was about to explain that her parents called her Meghan. Hence they are responsible for this, not her. Indeed there is no trail that suggests Meghan was ever once widely known as Rachel and that it was her who radically decided to change the name everyone knew her by. She always been known as Meghan! Either way Rachel is equally a cool name too. It’s not a slur or a name people think is not nice. It is a clear example of parasocial behaviour when odd internet individuals come up with theories and project them onto Meghan like deciding she’s ashamed of Rachel and deciding that there’s some sinister reason for why she’s been called Meghan since forever. Indeed there is no trail that suggests Meghan was ever once widely known as Rachel and that it was her who radically decided to change the name everyone knew her by. She always been known as Meghan! American Surgeon shows the actual letter from UnitedHealthcare DENYING a patient in emergency condition from receiving care “This is a woman who was in the emergency room with pulmonary embolisms” “I think we all knew this would happen. I had another patient come in and share with me that UnitedHealthcare denied her inpatient's day. So this is a patient who had shortness of breath and some chest pain, and she just knew that something wasn't right in her body. She had a family history of blood clots and she'd had a deep flap surgery a couple of weeks ago. She went to the hospital and they saw her and they found that she had a life threatening condition known as pulmonary embolisms. So she was admitted to the hospital and taken care of really well by the doctors there. And they ordered all the right things. After a couple of days, she was discharged. She got a letter from UnitedHealthcare explaining that they didn't agree with the level of her care and that they would not cover it. So I'm gonna share some of the language of that letter with you, and I want you to know that my patient that we talked about previously who had her surgery denied had almost exactly the same letter shared. So there's some troubling things in this letter. I think this term is really interesting. United is saying they reviewed the request for inpatient admission. So let's all just pause and consider that. This is a woman who was in the emergency room with pulmonary embolisms, and the doctor wasn't really requesting anything. They were saying this patient needs to be in the hospital. But an insurance company sees this as a request, and that's part of this prior auth environment that we're living in. So I think it's important as patients and as physicians to just acknowledge that this is our reality now. Someone can think that there's a good medical decision for you and can write orders and wanna do the right thing for you, but your insurance company is seeing that as a request and deciding whether or not they wanna do it. One of the criteria that this insurance company used to decide whether or not to accept or deny this request was whether it's medically necessary. And it's so interesting that we're letting insurance companies and the doctors who work for insurance companies determine what's medically necessary and not just the doctor in front of the patient in the emergency room. So this is a really bold statement from UnitedHealthcare for my patient. They say you did not have to be admitted as an inpatient to the hospital for this care. I think we all need to just reflect on that. An insurance company is telling a patient and her doctor that they disagree with the plan of care to keep that patient safe. I genuinely cannot understand how someone can watch this story and still stand there, looking at two women, and somehow decide that the wrong one is the victim. On one side, you have a girl (Yıldız) who has been mistreated her entire life.
