Failure No. 6. Casting an ancient precision vase in an empty eggshell. The two ostrich eggs are patiently waiting in the background while I struggle with the cheap temu plastic eggs to fine tune the process. Today’s failure: I pushed a plastic cup into the goo from the top to make the carving out phase easier but it stuck in and I can’t remove it without risking the vase itself. Anyway, the next failure (failure No 7) will be different and I’ll reach my goal no matter what.

OTD in 1972: the DC-10 cargo door warning sign that went unnoticed and led to the worst crash in history June 12 1972: Windsor incident (Canada). American Airlines Flight 96, a DC-10, had an explosive decompression over Windsor, which damaged the controls. Showing extraordinary skills, the crew was able to perform an emergency landing in Detroit. And all 67 aboard are unhurt. The cause is later traced to a faulty cargo door design that had already failed in testing. Despite MDC initiating some actions to rectify the door, these turn out to be inefficient: a lack of decisive action on this issue will cause a Turkish Airways DC-10 to have a similar decompression over Paris in February 1974. This time, the persons aboard weren´t so lucky: the explosion caused total loss of control, and the aircraft plunged into Ermenonville Forest at over 450 Knots. All 346 aboard were killed in what was, at the time, the worst aviation disaster in history. A curious note on the Windsor accident was that, when the cargo door failed, a coffin was ejected from the cargo hold. A farmer in Ontario later found the said remains. Also on the American Airlines incident, due to their airmanship, the flight crew, Bryce McCormick (Captain), R. Paige Whitney (F/O), Clayton Burke (F/E), as well as the cabin crew, were all awarded the American Airlines Distinguished Service Award for Merit.

On 13.05.2026, Customs officers at IGI Airport, New Delhi intercepted two Indian male passengers arriving from Jeddah by flight no. SV-756 and Doha by flight no. 6E-1308 after they crossed the Green Channel. During detailed examination, Customs officers unearthed an ingenious modus operandi wherein water bottles had been specially modified with fake bottoms concealing circular pieces of raw gold coated with silver colour and meticulously crafted to fit inside, making detection extremely difficult during routine checks. The aircraft took off from Zhukovsky International Airport near Moscow at 10:35 local time, landed at Kubinka Air Base at 11:09, and then departed for a test flight at 11:14. Four minutes into the test flight, the crew reported a fire in the no.2 (right) engine. Video evidence of the flight’s final moments shows the aircraft with a severe fire in the no.2 engine, gradually rolling to the right while losing altitude. The available seat was positioned between two children who were not part of their family and the crew was simply trying to find the least disruptive solution for everyone on board. Here is the thing - the father himself had a simple option right in front of him. Stay seated next to his own child and let his wife take the middle seat. Problem solved, no argument, no disruption to an entire plane full of people. Planning your seats before you board is part of traveling with a family and turning a solvable seating issue into a scene at 30,000 feet is never the answer. On landing, the jet overran the runway and hit airport structures. Factors: crew actions – continuing approach in bad weather, improper use of spoilers, etc- fatigue, poor CRM, others (as detailed below) From the NTSB report - (...) "The probable causes of this accident were the flight crew's failure to discontinue the approach when severe thunderstorms and their associated hazards to flight operations had moved into the airport area, and the crew's failure to ensure that the spoilers had extended after touchdown. Contributing to the accident were the flight crew's (1) impaired performance resulting from fatigue, and the situational stress associated with the intent to land under the circumstances; (2) continuation of the approach to a landing when the company's maximum crosswind component was exceeded; and (3) use of reverse thrust greater than 1.3 engine pressure ratio after landing."
