Short answer: US Healthcare and patent laws suck. In countries with universal healthcare, chemotherapy and emergency medications like epis are often provided at little or no direct cost because the government acts as a single payer, negotiating prices and spreading costs across the population through taxes. The US however, chose a system where employers, insurers, and individuals bear much of the cost, and government intervention in drug pricing is VERY limited. Also, patenting plays a key role for both because patents and patent extensions give pharmaceutical companies temporary monopolies, allowing them to charge high prices without competition even when the underlying medicine is inexpensive or long established. In universal systems, governments counter this patent power through price controls and bulk purchasing, but in the US patent protection exists without strong price regulation, so “temporary monopolies” are not temporary at all. Covid vaccines were an exception because the government treated them as a public emergency and paid billions upfront using taxpayer money to fund research & development, manufacturing, and distribution, which allowed the vaccines to be given out free at the point of use.
