The dentist first isolates the tooth - often using a rubber dam - to prevent contamination by saliva, because moisture can compromise both the diagnosis and the bonding of filling materials. They remove decayed tissue by mechanically excavating the softened dentin with a high-speed or slow-speed bur; interestingly, newer research shows that selectively removing only infected dentin (and preserving affected but remineralizable dentin) reduces the risk of pulp exposure and maintains tooth vitality longer. Lasers or air abrasion devices may sometimes replace drills, minimizing microfractures in enamel. Once decay is cleared, the dentist etches the prepared surface with a mild acid, creating microscopic pores in the enamel and dentin to enhance adhesive penetration. A bonding agent is then applied, chemically linking the filling to the tooth’s collagen fibers at a molecular level. In composite fillings, each layer of resin is cured with blue light (photo-polymerization) to minimize polymerization shrinkage, which - if uncontrolled - can cause microleakage and future decay under the filling. Throughout, dentists assess subtle color changes in dentin and enamel to gauge the depth and health of the remaining tissue, a skill not easily visible to patients but critical for long-term success.
