It’s true. This was practiced in the 1500s. After being pumped with smoke, it would cause painful gas which is where we got the phrase we still use today, “fire in the hole”.

Tobacco enemas were used by Western medical practitioners as a remedy for colds and lethargy. The procedure was used to treat intestinal pain and was often used to revive drowning victims. Liquid tobacco enemas were often given to relieve hernia symptoms. The practice began to decline in the early 19th century when nicotine, the primary active ingredient in tobacco smoke, was discovered to be poisonous.

Rooted in the humoral theory and the idea that stimulating the intestines could revive life or correct internal imbalances, it was widely used in Europe and colonial America, particularly along the River Thames, where kits were stationed for near-drowning victims. The physiological rationale - though primitive by modern standards - centered on nicotine’s parasympatholytic effects, which could increase heart rate, stimulate respiration, and promote peristalsis. The practice gained traction partly due to Native American observations, where tobacco had ritual and medicinal uses, though not via rectal administration. The Royal Humane Society even officially endorsed the technique, funding resuscitation kits, until more refined understanding of respiratory physiology and the advent of artificial respiration rendered it obsolete. Ironically, the eventual realization that nicotine was a toxic alkaloid with dangerous systemic effects helped end its medical use, marking one of the first instances of iatrogenic harm influencing mainstream medical abandonment.
