Diagnosis: Geographic Atrophy secondary to Dry Age-Related Macular Degeneration (AMD) Why: The fundus photos show large, well-defined, confluent areas of yellowish-orange RPE/choriocapillaris atrophy with visible underlying choroidal vessels. That map-like pattern is classic for Geographic Atrophy (GA) - the advanced form of dry AMD.

Cotton wool spots are retinal microinfarcts seen as fluffy white patches, most often linked to diabetes, hypertension, or vascular occlusion.

Dx:Cotton-wool spots in diabetic retinopathy. The fundus shows multiple soft, fluffy, yellowish-white patches on the retina, which are cotton-wool spots (soft exudates). They occur because long-term diabetes damages the small retinal blood vessels. Some vessels become blocked, reducing blood flow and oxygen to the retina. This causes tiny nerve fibre layer infarctions with accumulation of axoplasm, forming the white cotton-wool spots. Mgt; 👉 Improve blood sugar, blood pressure, and cholesterol control 👉perform regular eye examinations, and treat advanced disease with anti-VEGF injections, laser photocoagulation, or vitrectomy if complications occur.
